搞要目的探讨基于家系的系统性红斑狼疮(SLE)患者的实验室及临床特点,为SLE个体化诊治及其病因研究提供依据。方法分析SLE家系3位患者的临床实验室检测指标,利用多个检测指标(抗核抗体ANA阳性,类风湿因子RF阳性及免疫球蛋白Ig E阳性)建立筛选类似SLE家系(简称“类家系”)患者模型ANA+RF+Ig E+,按此模型筛选近4年华西医院确诊的SLE患者,分析该类患者与其他SLE患者的差异。结果从877例SLE患者中筛选类家系ANA+RF+Ig E+患者55例(6.27%);类家系SLE患者血肌酐、白细胞、C反应蛋白水平均显著低于其他SLE患者,而血Ig G、抗SSA和抗SSB阳性率则显著升高(P<0.05);临床表现方面,类家系患者脱发、皮疹的比例显著高于其他SLE患者(P<0.05)。结论类家系ANA+RF+Ig E+的SLE患者炎性程度较低;累及肾脏功能较轻;临床症状以脱发和皮疹表现较为突出,该类SLE亚型患者值得进一步关注和研究。
Objective To investigate the laboratory and clinical features of pedigree-based systemic lupus erythematosus (SLE) in order to provide a basis for the individual diagnosis and treatment of SLE and its etiology. Methods The clinical laboratory parameters of 3 patients with SLE pedigree were analyzed. A number of SLE pedigrees (referred to as “pedigree ”) were screened by using multiple detection indexes (anti-nuclear antibody ANA positive, rheumatoid factor RF positive and immunoglobulin Ig E positive) ") Patients with ANA + RF + Ig E +, according to this model screening nearly 4 years West China Hospital confirmed SLE patients, analysis of these patients and other patients with SLE differences. Results Among 877 SLE patients, 55 cases (6.27%) were ANA + RF + Ig E + patients. The levels of serum creatinine, leukocyte and C-reactive protein in SLE patients were significantly lower than those in other SLE patients, The positive rate of anti-SSA and anti-SSB was significantly increased (P <0.05). In clinical manifestations, the percentage of alopecia and rash in pedigree patients was significantly higher than that in other SLE patients (P <0.05). Conclusions SLE patients with pedigree ANA + RF + Ig E + have lower degree of inflammation, less renal involvement and more obvious clinical manifestations of hair loss and rash. These SLE patients deserve further attention and research.
