目的 比较实施每年 30 %和 5 0 %人群筛检化疗两种方案的费用与效果 ,提出控制疾病的优化化疗方案。 方法 在鹤庆县的古乐和新庄两行政村中 ,选择 12个自然村 ,随机分为A、B两组。A组每年对 6~ 6 0岁人群用间接血凝试验 (IHA)筛检 30 % ,连续 3年 ;B组每年筛检 5 0 % ,连续 2年 ;IHA≥ 1∶10者用吡喹酮化疗。分析两方案的费用与效果。 结果 A组居民感染率下降率、病人感染率下降率、人群感染率下降率、钉螺自然感染率下降率、感染螺密度下降率分别为 45 37%、14 5 4%、36 79%、2 9 6 7%、9 5 2 % ;B组上述指标分别为 39 81%、15 49%、2 3 91%、10 0 0 %、3 5 4%。 结论 两种筛检化疗方案均可作为中度疫区控制疾病的防治方案 ,其效果以每年筛检 30 %化疗为佳
Objective To compare the cost and effect of two programs of 30% and 50% screening and chemotherapy annually, and to propose an optimized chemotherapy regimen for disease control. Methods In the villages of Gule and Xinzhuang of Heqing County, 12 natural villages were selected and randomly divided into A and B groups. A group of 6 ~ 60 years of age each year with indirect hemagglutination test (IHA) screening 30% for 3 consecutive years; B group screening 50% every year for 2 consecutive years; IHA ≥ 1:10 with praziquantel Chemotherapy. Analysis of the cost and effectiveness of the two programs. Results In group A, the rate of decline of infection rate, the rate of decrease of infection rate, the rate of decrease of population infection rate, the decrease rate of spontaneous infection rate of snails and the decrease rate of infected snail density were 45 37%, 14 5 4%, 36 79% and 29 6 7% and 95 2% respectively. The above indicators in Group B were 39 81%, 15 49%, 23 91%, 100% and 345 4% respectively. Conclusion Both of the two screening and chemotherapy regimens can be used as a control program to control the disease in moderate epidemic areas, and the effect of which is to screen 30% of chemotherapy annually
