目的应用24小时动态心电图检测2型糖尿病合并冠心病患者的心率变异性。方法选取42例2型糖尿病合并冠心病患者为观察组研究对象,30例健康志愿者为对照组,对所有研究对象进行24小时动态心电图检查,比较其心率变异性指标。结果观察组和对照组平均心率比较差异无统计学意义(P>0.05)。观察组患者睡眠状态心率变异性时域指标与清醒状态时比较差异无统计学意义(P>0.05)。对照组睡眠状态心率变异性时域指标均高于清醒状态时,比较差异有统计学意义(P<0.05)。结论 24小时动态心电图测定心率变异性是一项无创性检测心脏自主神经张力的技术,可用于2型糖尿病合并冠心病患者自主神经系统损伤的评估。
Objective To evaluate the heart rate variability of patients with type 2 diabetes mellitus complicated with coronary heart disease by 24-hour ambulatory electrocardiogram. Methods Forty-two patients with type 2 diabetes mellitus with coronary heart disease were selected as observation group and 30 healthy volunteers as control group. All subjects were examined by 24-hour Holter monitoring and their heart rate variability was compared. Results There was no significant difference in average heart rate between observation group and control group (P> 0.05). There was no significant difference in the time domain index of sleep state heart rate variability between observation group and awake state (P> 0.05). The time-domain indexes of heart rate variability of sleep state in control group were all higher than those in awake state, the difference was statistically significant (P <0.05). Conclusion 24-hour Holter monitoring of heart rate variability is a noninvasive technique to detect cardiac autonomic nervous system. It can be used to evaluate autonomic nervous system injury in patients with type 2 diabetes mellitus and coronary heart disease.
