非糠皮质激素治疗新生儿缺氧缺血性脑病89例临床分析

目的 探讨在新生儿缺氧缺血性脑病(HIE)综合治疗中是否需要用糖皮质激素。方法 本文就我院1997年1月至1998年11月收治174例HIE患儿随机分组:治疗组在综合治疗基础上加用地塞米松,观察组未用糠皮质激素。结果 治疗组病死率高于观察组,但经统计学处理:(X~2=0.62,P>0.05)差异不显著。而并发消化道应激性溃疡出血,治疗组明显高于观察组,经统计学处理:(X~2=4.82,P<0.05)差异显著。结论 糖皮质激素在HIE治疗中并不能降低病死率,而其不良反应较多,利少弊多,主张不予使用。

Objective To investigate whether glucocorticoid is needed in the treatment of neonatal hypoxic-ischemic encephalopathy (HIE). Methods In our hospital from January 1997 to November 1998 174 cases of children with HIE were randomly divided into: the treatment group on the basis of comprehensive treatment with dexamethasone, the observation group without the use of the glucocorticoid. Results The mortality of the treatment group was higher than that of the observation group, but the difference was not statistically significant (X ~ 2 = 0.62, P> 0.05). However, the bleeding of peptic ulcer complicated by digestive tract was significantly higher in the treatment group than in the observation group. The difference was statistically significant (X ~ 2 = 4.82, P <0.05). Conclusion Glucocorticoids can not reduce the mortality in the treatment of HIE, and its adverse reactions are more, the advantages and disadvantages are less and more, advocates not to use.
