目的:探讨5-FU宫颈局部注射对CIN的阻断逆转效果及远期疗效评价。方法:5-FU3ml(75mg)分点局部注射,深度2～4mm,隔日1次,10次为1疗程,每疗程间隔7～10日。结果:治疗前经病理组织学诊断为CIN24例,其中CINⅠ级12例。经1～3个疗程治疗,6例经病理证实恢复正常;1例7年后行锥切术,病理检查仍可见CINⅠ级改变;1例在继续注射中。治愈率CIN-Ⅱ级可达100%,CIN-Ⅲ为60%,所有CIN-Ⅲ级病例经治疗全部降低级别。4例HPV检测阳性者中3例转阴,1例效价下降接近正常值。结论:5-FU局部注射治疗CIN疗效可靠,方法简便,无创伤性,能保留完整宫颈及正常功能,远期随访复发率低,无恶变发生,适于未婚未育者。
Objective: To investigate the reversal effect and long-term curative effect of 5-FU cervical intracapsular injection on CIN. Methods: 5-FU3ml (75mg) points local injection, the depth of 2 ~ 4mm, every other day, 10 times for a course of treatment, each treatment interval of 7 to 10 days. Results: Before treatment, 24 cases were diagnosed as CIN by histopathology, including 12 cases of CIN Ⅰ grade. After 1 to 3 courses of treatment, 6 cases returned to normal by pathological examination. One case had condyctomy 7 years later. CIN Ⅰ level change was still observed in pathological examination. One case continued injection. CIN-Ⅱ grade cure rate of up to 100%, CIN-Ⅲ was 60%, all CIN-Ⅲ-class cases were treated to reduce all levels. In 4 cases of HPV test positive, 3 cases turned negative, and one case decreased titer close to normal. Conclusion: The local injection of 5-FU for the treatment of CIN is reliable, the method is simple, noninvasive, can retain the complete cervix and normal function, long-term follow-up low recurrence rate, no malignant transformation occurs, suitable for unmarried offspring.
