目的 为探讨慢性粒细胞性白血病 (CGL)骨髓组织活检分型及其与临床分期、预后的关系。方法 按 Bartl及 Frisch分型及参照 Hannover分类法 ,分析组织学特征与临床的关系。结果 粒细胞 /巨核细胞 (GRAN/MEG)发生纤维化 (MF)比率高、中位生存期 2 2个月 ,比粒细胞型 (GRAN) 4 8个月明显要短。出现幼稚前体细胞异位 (AL IP) 9例 ,其中 8例为加速期及慢性晚期。结论 骨髓活检可作为临床分期及预后参考 ,并可补充及修正临床分期之不足。
Objective To investigate the classification of bone marrow biopsy in patients with chronic myelogenous leukemia (CGL) and its relationship with clinical stage and prognosis. Methods According to Bartl and Frisch classification and Hannover classification, the relationship between histological features and clinical data was analyzed. RESULTS Granulocyte/megakaryocyte (GRAN/MEG) had a high rate of fibrosis (MF) and a median survival of 22 months, which was significantly shorter than that of granulocyte-type (GRAN) at 48 months. There were 9 cases of naive precursor ectopic (AL IP), 8 of which were accelerated and chronically advanced. Conclusions Bone marrow biopsy can be used as a reference for clinical stage and prognosis, and can supplement and correct the lack of clinical staging.
