目的评价多层螺旋CT灌注成像对乳腺肿块的诊断价值。方法对17例乳腺肿块性病变患者(22个病灶)行多层螺旋CT电影模式灌注扫描,其中乳腺癌9例,良性肿块8例。对生成的动态图像数据进行测量分析,包括血流量(BF)、血容量(BV)、平均通过时间(MTT)、血管表面通透性(PS),所得数据按乳腺癌与良性肿块分组,并进行统计学分析。结果乳腺癌组BF(35.92±8.36)mL/(min.100g),BV(12.79±3.13)mL/100g,MTT(15.37±4.45)s,PS(19.72±5.08)mL/(min.100g);良性肿块组BF(16.83±5.36)mL/(min.100g),BV(5.35±2.43)mL/100g,MTT(16.54±4.19)s,PS(5.47±2.83)mL/(min.100g),两组BF、BV、PS之间的差异有统计学意义(P<0.05)。结论乳腺癌的血流量、血容量和毛细血管通透性明显高于良性肿块,有助于乳腺癌的诊断和鉴别诊断。
Objective To evaluate the diagnostic value of multislice CT perfusion imaging in breast masses. Methods Seventeen patients with breast lesions (22 lesions) underwent multi-slice CT perfusion scanning. Among them, 9 were breast cancer and 8 were benign tumors. The generated dynamic image data were analyzed including blood flow (BF), blood volume (BV), mean transit time (MTT) and vascular surface permeability (PS). The data were grouped according to breast and benign masses Statistical analysis. Results The breast cancer group had significantly higher BF (35.92 ± 8.36) mL / (min.100g), BV (12.79 ± 3.13) mL / 100g, MTT (15.37 ± 4.45) s and PS (19.72 ± 5.08) mL / BF (16.83 ± 5.36) mL / (min.100g), BV (5.35 ± 2.43) mL / 100g, MTT (16.54 ± 4.19) s and PS (5.47 ± 2.83) mL / The difference between BF, BV and PS was statistically significant (P <0.05). Conclusion Breast cancer blood flow, blood volume and capillary permeability were significantly higher than the benign mass, contribute to the diagnosis and differential diagnosis of breast cancer.
