目的观察小剂量托吡酯治疗婴儿良性部分性癫痫的疗效。方法对大连市儿童医院2009年9月至2010年9月收治的47例婴儿良性部分性癫痫的患儿应用托吡酯单药治疗,观察发作情况及进行停药后随访。结果男27例,女20例。首发年龄:3~24个月,平均(10.0±4.9)个月。接受治疗前发作次数:3~20次,平均发作(6.1±3.8)次。接受治疗前病程10 d至3个月,平均病程(21.7±19.0)d。47例患儿均采用托吡酯单药治疗,最小剂量1.5 mg/(kg·d),最大剂量3 mg/(kg·d),平均治疗剂量(1.92±0.29)mg/(kg·d)。托吡酯单药治疗时间:最短3个月,最长13个月,平均治疗时间(7.66±2.33)个月。47例患儿应用托吡酯治疗剂量加至平均治疗剂量后均未再发作。停药后随访6~18个月,平均停药后随访(12.09±2.32)个月。47例患儿停药后均未复发。结论小剂量、短疗程托吡酯用于治疗婴儿良性部分性癫痫可以取得比较理想的治疗效果。
Objective To observe the curative effect of low dose topiramate on benign partial epilepsy in infants. Methods 47 cases of benign partial partial epilepsy in infants admitted to Dalian Children ’s Hospital from September 2009 to September 2010 were treated with topiramate monotherapy. The seizures were observed and followed up. Results There were 27 males and 20 females. The starting age: 3 to 24 months, an average of (10.0 ± 4.9) months. The number of seizures before treatment: 3 to 20 times, the average attack (6.1 ± 3.8) times. Before treatment, the course of disease ranged from 10 d to 3 months with an average course of 21.7 ± 19.0 d. All the 47 patients were treated with topiramate monotherapy. The minimum dose was 1.5 mg / (kg · d), the maximum dose was 3 mg / (kg · d) and the average therapeutic dose was 1.92 ± 0.29 mg / (kg · d). Topiramate monotherapy time: the shortest 3 months, up to 13 months, the average treatment time (7.66 ± 2.33) months. 47 cases of children treated with topiramate dose to the average treatment dose were no longer episodes. The patients were followed up for 6-18 months after discontinuation of drug, and were followed up for a mean of 12.09 ± 2.32 months. None of the 47 children relapsed after stopping. Conclusion Small dose, short course of topiramate for the treatment of benign partial infant epilepsy can obtain the ideal therapeutic effect.
