目的探讨临床诊断用ABR仪通过加载Smart Screen程序进行新生儿蜗后病变筛查的可行性。方法 2006年6月至2009年10月在解放军第309医院和解放军第316医院妇产科出生的2864例新生儿中的550例在出生后42d后接受AABR筛查,用美国智听公司诊断用ABR仪加载Smart Screener测试程序进行。AABR采用35dBnHL的短声刺激,刺激率为19.3次/s,叠加500-2000次,在标准隔声室内进行测试。对于筛查不通过者转诊至上级听力诊断中心进行听力评估。结果 550例新生儿中535例通过了初筛,筛查通过率97.27%(535/550),所有测试未通过者转往北京市指定的听力筛查中心,转诊百分比5.24‰(15/2864),诊断听力正常12例,确诊感音神经性聋、先天性小耳畸形和唇腭裂各1例次,占同期出生新生儿的1.05‰(3/2864)。AABR筛查通过的535例婴儿,随访半年以上,听力发育未见明显异常。结论诊断用ABR仪加载Smart Scree-ner测试程序不需要添加额外设备,利用产妇产后42d到医院复查时对婴儿进行筛查,如发现筛查异常还可立即进行诊断性ABR检查,具有一定的推广应用价值。
Objective To investigate the feasibility of using ABR instrument to screen neonatal cochlear lesions by loading Smart Screen program in clinical diagnosis. Methods From June 2006 to October 2009, 550 out of 2864 newborns born in the 309th Hospital of PLA and the 316th Hospital of People’s Liberation Army were enrolled in AABR screening 42 days after birth and were diagnosed ABR loaded Smart Screener test program. AABR using 35dBnHL short-tone stimulation, the stimulation rate of 19.3 times / s, superimposed 500-2000 times, in a standard room to test noise. For screening failed referral to superior hearing diagnosis center for hearing assessment. Results Among the 550 neonates, 535 cases passed the screening test. The screening pass rate was 97.27% (535/550). All those who failed the test were transferred to the designated hearing screening center in Beijing. The referral percentage was 5.24% (15/2864) ). There were 12 cases diagnosed as normal hearing, 1 case diagnosed with sensorineural hearing loss, congenital malformations and cleft lip and palate, accounting for 1.05 ‰ (3/2864) of newborns born in the same period. AABR screening through 535 infants, followed up for more than six months, no significant abnormal hearing development. Conclusion The diagnostic ABR loaded Smart Scree-ner test program does not require additional equipment, the use of maternal postpartum 42d to the hospital for screening during screening, found that abnormal screening can be diagnosed immediately ABR examination, with some promotion Value.
