Cavernous vascular malformation, also known as cavernous hemangioma, is a rare disease common in the cerebral hemispheres, rare in the brainstem, basal ganglia, periventricular. These patients often have hydrocephalus, memory loss, hypothalamic dysfunction symptoms and signs. There were 15 cases of triple ventricular cavernous hemangiomas in the past, but only 3 cases were evaluated by MRI. Four rare cases of this kind were reported in this paper and CT and MRI data were discussed. Case 1, female, 43 years old, intermittent headache for 2 weeks, nervous system examination except for a slight memory loss outside the normal. CT showed diffuse high-density shadow -3 cm mass filled Ⅲ ventricle, mild homogeneity, enlargement of the lateral ventricle, MRI T_1, T_2 weighted images were mixed signals, contrast enhanced slightly, lesions completely in the third ventricle, Ⅲ enlarged ventricles , The purpose of surgery for further diagnosis and ventricular decompression through the corpus callosum fornary approach, the surgical choice is because Monro’s hole did not expand, Ⅲ ventricular base becomes polar
