目的应用三维斑点追踪技术(3D-STI)评价2型糖尿病(T2DM)患者左室整体收缩功能,及其与颈动脉内中膜厚度(IMT)的相关性分析。方法收集65例T2DM患者(病例组),根据HbA1c水平分为DM1组30例(HbA1c<7%)和DM2组35例(HbA1c≥7%),健康对照组40例,采用3D-STI技术获取各组左心室整体纵向收缩期峰值应变(GLS)、径向应变(GRS)、圆周应变(GCS)、面积应变(GAS)、左心室射血分数(LVEF),二维测量IMT,比较各组指标间的差异,并分析各应变值与IMT的相关性。结果与对照组相比,DM1组GLS降低,与对照组、DM1组相比,DM2组GLS、GAS、GCS及GRS降低,IMT在3组中逐渐增厚,差异均有统计学意义。GLS、GAS、GCS及GRS值与IMT呈负相关。结论 3D-STI可以准确地评价左室收缩功能,为临床提供可靠依据。
Objective To evaluate the global left ventricular systolic function in patients with type 2 diabetes mellitus (T2DM) and its correlation with carotid intima-media thickness (IMT) using 3D speckle tracking (3D-STI). Methods Totally 65 patients with T2DM (case group) were enrolled in this study. According to the HbA1c level, 30 patients (HbA1c <7%) in DM1 group and 35 patients (HbA1c≥7%) in DM2 group were enrolled in this study. The left ventricular total longitudinal systolic peak strain (GLS), radial strain (GRS), circumferential strain (GCS), area strain (GAS), left ventricular ejection fraction (LVEF) Differences between the indicators, and analyze the correlation between strain values and IMT. Results Compared with the control group, GLS decreased in DM1 group. Compared with control group and DM1 group, GLS, GAS, GCS and GRS decreased in DM2 group and gradually increased in 3 groups. The difference was statistically significant. GLS, GAS, GCS and GRS values were negatively correlated with IMT. Conclusion 3D-STI can accurately evaluate left ventricular systolic function and provide a reliable basis for clinical practice.
