[目的]比较双横连与单横连长节段固定治疗胸腰段骨折-脱位的临床疗效.[方法] 2011年6月~2014年9月,53例胸腰段骨折-脱位患者行后路切开复位长节段内固定术,其中27例选择双横连,26例选择单横连.记录两组术后伤椎节段Cobb角、伤椎前缘高度压缩率、内固定并发症、JOA功能评分等,并进行统计学比较.[结果]两组手术时间、术中出血量、术后引流量、围手术期并发症、卧床时间差异无统计学意义(P>0.05).所有患者平均随访时间为15.9个月,术后佩戴支具时间、JOA功能评分差异无统计学意义(P>0.05).两组术后末次随访伤椎节段Cobb角、伤椎前缘高度压缩率差异有统计学意义(P<0.05).双横连组无内固定并发症,单横连组出现断棒1例,断钉1例,钉棒松动3例,两组间并发症发生率差异有统计学意义(P<0.05).[结论]对胸腰段骨折-脱位行切开复位长节段内固定时,应采用双横连为宜.“,”[Objective] To explore the clinical efficacy of double versus single transverse cross-link in long segment pedicle screw-rod systems for fixation of thoracolumbar fracture-dislocation.[Methods] From June 2011 to September 2014,53 patients with thoracolumbar fracture-dislocation were treated by open reduction and internal fixation with long segment pedicle screw-rod systems.Of them,21 patients received double transvers cross-links in the system (the double-link group),whereas the other 26 patients had only a single transvers cross-link (the single-link group).Clinical data such as JOA score and complication,additionallythe Cobb angle and compression rate of anterior vertebral body heightmeasured by X-ray film were compared between the two groups.[Results] There were no significantly statistical differences in operation time,blood loss,postoperative drainage,bed rest time and perioperative complications between the two groups (P>0.05).All the patients were followed up for an average of 15.9 months.No statistical differences were noted in time using brace and postoperative JOA scores between them (P > 0.05).However,the double-link group was statistically superior to the single-link group in imaging measurements,including improvement of Cobb angle and vertebral compression rate (P<0.05).In addition,no failure of internal fixation was noted in any patient in the double-link group,whereas rod broken in 1 patient,screw broken in 1 and screw rod loosening in 3 were found in the single-link group at 9 months and 12 months after surgery,associated with statistically significant difference between the two groups (P<0.05).[Conclusion] It had better use double cross-links in long segment pedicle screw-rod fixation for treatment of thoracolumbar fracture-dislocation.
