目的探讨老年慢性肺源性心脏病(肺心病)患者肺部非发酵菌感染和耐药情况。方法对老年肺心病患者的痰培养分离出的非发酵菌种类及药敏情况进行分析。结果126例老年肺心病患者痰标本共分离出非发酵菌107株,其中铜绿假单胞菌55株,鲍曼/溶血不动杆菌28株,嗜麦芽窄食单胞菌15株。药敏结果显示,非发酵菌对第三代头孢菌素的耐药已十分显著,平均耐药率达50%~100%;含酶抑制剂复方型抗菌药物特美汀、舒普深、亚胺培南及左氧氟沙星耐药率低(3.6%~35.7%),与三代头胞及其他抗菌药物相比差异有显著性(P<0.05)。结论老年肺心病患者肺部非发酵菌感染耐药率高,宜选用复合抗菌药物对非发酵菌感染者进行的治疗。
Objective To investigate the pulmonary non-fermentive bacterial infections and drug resistance in elderly patients with chronic cor pulmonale (pulmonary heart disease). Methods The types of non-fermentative bacteria isolated from sputum culture of elderly patients with pulmonary heart disease and their susceptibility were analyzed. Results A total of 107 non-fermentative bacteria were isolated from sputum of 126 elderly patients with pulmonary heart disease, including 55 Pseudomonas aeruginosa, 28 Bauman / Acinetobacter baumannii and 15 Stenotrophomonas maltophilia. Drug susceptibility results show that non-fermenting bacteria on the third-generation cephalosporin resistance has been very significant, the average resistance rate of 50% to 100%; containing inhibitor of the compound antimicrobial drugs timentin, Shu Pu deep, Asia The rates of resistance to amipenem and levofloxacin were low (3.6% -35.7%), which were significantly different from those of the third generation of cephalic cells and other antibacterials (P <0.05). Conclusion Pulmonary non-fermentive bacterial infections in elderly patients with pulmonary heart disease have high rates of resistance. It is advisable to treat patients with non-fermenter infections with composite antibacterials.
