目的:探讨高频彩色多普勒超声检查评估颈动脉内膜中层厚度(IMT)与冠状动脉粥样硬化的相关性。方法:选择60岁以上发生胸闷的住院患者206例,给予冠状动脉造影(CAG)检查,根据检查结果分为冠状动脉粥样硬化组130例(冠状动脉单支病变50例,多支病变80例),冠状动脉正常组76例。同时采用高频超声测量颈动脉IMT及斑块形态。结果:冠状动脉粥样硬化组中,IMT增厚、动脉粥样硬化斑块形成94例,发生率72.3%(94/130);正常组中,颈动脉IMT增厚、动脉粥样硬化斑块形成18例,发生率23.7%(18/76)。两组发生率比较,差异显著(P<0.05)。冠状动脉多支病变伴颈动脉IMT增厚及斑块形成和钙化的发生率为77.5%(62/80),显著高于冠状动脉单支病变的30.0%(15/50,P<0.05)。结论:颈动脉IMT及斑块形成与冠状动脉病变具有一定相关性。
Objective: To investigate the correlation between carotid intima-media thickness (IMT) and coronary atherosclerosis by high-frequency color Doppler ultrasonography. Methods: 206 hospitalized patients with chest tightness over the age of 60 were selected for coronary angiography (CAG). According to the results of the examination, they were divided into coronary atherosclerosis group of 130 cases (coronary artery single vessel disease in 50 cases, multiple vessel disease in 80 cases ), 76 cases of normal coronary artery. At the same time, high frequency ultrasound was used to measure the carotid IMT and plaque morphology. Results: In coronary atherosclerosis group, IMT was thickened and atherosclerotic plaque was formed in 94 cases, the incidence rate was 72.3% (94/130). In the normal group, carotid IMT was thickened, atherosclerotic plaque 18 cases were formed, the incidence rate was 23.7% (18/76). The incidence of the two groups was significantly different (P <0.05). The incidence of multiple coronary artery disease with carotid IMT thickened, plaque formation and calcification was 77.5% (62/80), which was significantly higher than that of coronary artery single vessel disease (30.0%, 15/50, P <0.05). Conclusion: Carotid artery IMT and plaque formation have some correlation with coronary artery disease.
