乳果糖已广泛用于治疗肝性脑病,但乳果糖太甜,不易被某些患者接受。拉替妥(Lactitol)是另一种双糖,易溶于水,味觉良好,在小肠不被吸收,在治疗肝性脑病时可以替代乳果糖。作者用体外粪便孵化的方法比较了这两种糖的代谢,结果证实这两种糖都易被粪杆菌所代谢产生挥发性脂肪酸,而新霉素能抑制这种代谢。用遥测技术监测6例正常人回肠末端及结肠内pH,结果发现右下结肠用药前pH为6.51±0.48,用
Lactulose has been widely used to treat hepatic encephalopathy, but lactulose is too sweet, not easily accepted by some patients. Lactitol is another disaccharide, soluble in water, taste good, is not absorbed in the small intestine, and can replace lactulose in the treatment of hepatic encephalopathy. The authors compared the metabolism of these two sugars with in vitro faecal incubation and found that both sugars were easily metabolized by the bacterium to produce volatile fatty acids that neomycin inhibited.
