目的:分析高原正常成人多普勒超声心动图右心系统及血流动力学改变特点。方法:选取世居高原正常成人50例(高原世居组)和移居高原正常成人50例(高原移居组)为研究对象,另外选取平原健康成人为对照组(平原组),采用多普勒超声心动图及心电图追踪检测心功能和心脏结构变化。采用单因素方差分析对三组的各个指标进行差异性检验。结果:高原世居组和高原移居组心脏结构参数、右心血流动力学变化无明显差异(P>0.05),高原世居组和高原移居组左室收缩末期内径(LVDs)、舒张末左室后壁厚度(LVPWd)、右室舒张末期最大径显著大于平原组(P<0.05)。高原世居组和高原移居组右室流出道、左房、右心室前壁指标显著大于平原组(P<0.05)。高原世居组肺动脉平均压(PAMP)(27.3±2.3)mm Hg和右室射血前期时间(RVPEP)(127.4±4.9)ms、高原移居组PAMP(27.4±2.5)mm Hg和RVPEP(128.5±5.6)ms显著大于平原组PAMP(20.1±2.8)mm Hg和RVPEP(90.6±6.4)ms(P<0.05),高原世居组肺动脉加速时间(AT)(113±8.4)ms和高原移居组(115±6.2)ms显著小于平原组(137±9.4)ms(P<0.05)。结论:高原正常成人PAMP、AT改变,右心系统测量值增大,建议移居人员定期健康体检,减少器质性改变和减损。
Objective: To analyze the changes of right heart system and hemodynamics in normal adult Doppler echocardiography. Methods: 50 healthy adults (plateau group) and 50 normal adults (plateau migrant group) were selected as experimental subjects. In addition, plain healthy adults were selected as the control group (plain group) and Doppler ultrasound Cardiogram and electrocardiogram tracking to detect cardiac function and cardiac structural changes. One-way ANOVA was used to test the differences among the three groups. Results: There were no significant differences in cardiac structural parameters and right ventricular hemodynamic parameters between the high altitude group and the highland group (P> 0.05). The left ventricular end-diastolic diameter (LVDs) The wall thickness (LVPWd) and right ventricular end-diastolic diameter were significantly larger than those in the plain group (P <0.05). The indexes of right ventricular outflow tract, left atrium and right ventricular anterior wall in the plateau group and plateau group were significantly higher than those in the plain group (P <0.05). The PAMP (27.3 ± 2.3) mm Hg, RVPEP (127.4 ± 4.9) ms, PAMP (27.4 ± 2.5) mm Hg and RVPEP (128.5 ± 5.6) ms was significantly higher than that of plain group (P <0.01), P <0.01 compared with RVPEP (90.6 ± 6.4) ms and P> 0.05 115 ± 6.2) ms was significantly less than that of plain group (137 ± 9.4) ms (P <0.05). CONCLUSIONS: PAMP, AT changes and right ventricular system measurements of normal adults in the plateau increase. It is suggested that migrants should take regular physical examination to reduce organic changes and impairments.
