刘某,女,68岁。因两手指疼痛、变形、活动障碍3年,头昏、乏力、面部浮肿1年,尿量减少2天,于1985年4月5日入院。患者逐渐出现两手指、两足趾关节疼痛、肿胀、晨僵,以后出现关节僵硬,活动障碍3年。曾多次来本院及外院就诊,拟诊“类风湿性关节炎”,给于抗风湿灵、消炎痛、保泰松等药治疗,症状无改善。近日来上述症状加重,尿量减少(500～600ml/天)。既往无肾脏疾病史,无引起尿毒症的疾病史。体检:面部、眼睑轻度浮肿,全身皮肤无瘀点、瘀斑、皮下结节、环形红斑及黄疸,贫血貌。
Liu, female, 68 years old. Due to two fingers pain, deformation, movement disorders 3 years, dizziness, fatigue, facial swelling 1 year, reduced urine output 2 days, on April 5, 1985 admission. Patients gradually appear two fingers, two toe joint pain, swelling, morning stiffness, later joint stiffness, movement disorders for 3 years. Has repeatedly come to this hospital and outside the hospital, diagnosed “rheumatoid arthritis”, to anti-rheumatism, indomethacin, phenylbutazone and other drug treatment, no improvement in symptoms. In recent days, the above symptoms, decreased urine output (500 ~ 600ml / day). No past history of kidney disease, no history of uremia. Physical examination: mild edema on the face and eyelids, no petechia on the whole body, ecchymosis, subcutaneous nodules, annular erythema and jaundice, anemia appearance.
