目的了解广东省深圳市不同性角色男男性行为人群(MSM)人口社会学和性行为特征及其影响因素,为探索针对性的干预活动提供有效信息。方法采用同伴推动抽样法招募MSM,对MSM进行匿名调查,调查内容包括社会人口学和高危性行为特征,同时采集血样进行HIV和梅毒抗体的血清学检测。结果共调查MSM 675人,据性角色不同分为三组,插入方(1号)180人(26.67%),被插入方(0号)132人(19.56%),兼有插入和被插入方(0.5号)363人(53.78%)。1号、0号、0.5号HIV感染率分别为12.22%、11.36%和12.12%,梅毒感染率分别为16.11%、21.21%和19.28%,差异均无统计学意义(P=0.97,P=0.49)。多因素结果显示25岁以下的男同人群中,1号的比例较大,是0号和0.5号的1.80倍(OR=1.80,95%CI:1.05~3.08);1号相对于0号和0.5号的首次性伴性别更倾向于男性(OR=2.62,95%CI:1.70~4.04)。结论不同性角色具有不同的人口学和性行为学特征,提示在该人群中进行艾滋病防治时,应根据不同亚群的特点,采取针对性的干预手段减少HIV等的传播。
Objective To understand the population sociology and sexual behaviors of MSM with different sexual roles and its influencing factors in Shenzhen, Guangdong Province, and to provide effective information for exploring targeted interventions. Methods A companion-driven sampling method was used to recruit MSM to conduct anonymous investigation of MSM. The investigation included social demography and high-risk sexual behaviors, and blood samples were collected for serological detection of HIV and syphilis antibodies. Results A total of 675 MSM were investigated and divided into three groups according to their sexual roles. 180 (26.67%) were inserted into party (No. 1) and 132 (19.56%) were inserted into party (No. 0) with both insertion and insertion (0.5 number) 363 people (53.78%). The prevalence of HIV infection in Nos. 1, 0 and 0.5 was 12.22%, 11.36% and 12.12% respectively, and the syphilis infection rates were 16.11%, 21.21% and 19.28% respectively, with no significant difference (P = 0.97, P = 0.49 ). Multivariate analysis showed that the proportion of No. 1 was larger in men younger than 25 years old, which was 1.80 times of that of No. 0 and No. 0.5 (OR = 1.80, 95% CI: 1.05-3.08); No. 1 was higher than No. 0 and Sex number 0.5 for the first time more inclined to male (OR = 2.62, 95% CI: 1.70 ~ 4.04). Conclusion Different sexes have different characteristics of demography and sexual behavior, suggesting that in the prevention and treatment of AIDS in this population, we should take targeted interventions to reduce the spread of HIV and so on according to the characteristics of different subpopulations.
