目的探讨糖化血红蛋白(HbA1C)在妊娠期糖尿病(GDM)血糖监测中的应用价值。方法将470例孕妇分为GDM组(100例)和正常孕妇对照组(470例),采用离子交换高效液相色谱分析法(HPLC)检测HbA1C,绘制ROC曲线,进行统计分析。结果 GDM组HbA1C(5.35±0.4)%高于对照组(5.03±0.3)%。ROC曲线分析表明,曲线下面积为0.78,以HbA1C5.2%作为诊断GDM的最佳切值(cut-off值)。结论HbA1C5.2%是诊断GDM的最佳cut-off值。HbA1C<5.2%可初步排除GDM,或表明GDM患者血糖控制良好。血糖联合HbA1C检测,可更好地对GDM进行筛查和血糖监控。
Objective To investigate the value of glycosylated hemoglobin (HbA1C) in the monitoring of blood glucose in gestational diabetes mellitus (GDM). Methods 470 pregnant women were divided into GDM group (n = 100) and normal pregnant women control group (n = 470). HbA1C was detected by ion exchange high performance liquid chromatography (HPLC), and the ROC curve was drawn for statistical analysis. Results HbA1C (5.35 ± 0.4)% in GDM group was higher than that in control group (5.03 ± 0.3)%. ROC curve analysis showed that the area under the curve was 0.78, and HbA1C 5.2% was the best cut-off value for diagnosing GDM. Conclusions HbA1C5.2% is the best cut-off value for diagnosis of GDM. HbA1C <5.2% may initially exclude GDM, or that GDM patients with good glycemic control. Blood glucose combined with HbA1C detection, can better GDM screening and blood glucose monitoring.
