目的探讨肺结核患者外周血CD26~+CD4~+T细胞百分含量和血清中sCD26水平变化。方法采取30例健康体检者(HD),30例非耐药初治肺结核患者(S-TB)及30例多耐药结核病患者(MDR-TB)2 ml肝素抗凝外周静脉血,经CD3、CD4、CD26流式抗体标记后,Beckman流式细胞仪(FCM)计数各组CD26~+CD4~+T细胞百分比含量,ELISA检测外周血清中sCD26水平。结果 MDR-TB组外周血CD26~+CD4~+T细胞占CD4~+T细胞总数的(13.63±3.85)%,低于S-TB组(20.17±5.94)%以及对照HD组(26.55±5.77)%,比较差异有统计学意义(F=45.037,P=0.000);MDR-TB组血清sCD26含量为(617.99±55.57)μg/ml,高于健康人群对照HD组含量(410.23±36.21)μg/ml以及S-TB组含量(435.22±38.34)μg/ml,比较差异有统计学意义(F=197.287,P<0.05)。Spearman相关分析,正常人群、S-TB和MDR-TB肺结核病患者CD26~+CD4~+T细胞百分比含量与血清sCD26水平变化之间存在负相关(r=-0.391,P=0.002)。结论随着肺结核由非耐药往耐药结核病发展,外周血中CD26~+CD4~+T细胞百分含量逐渐下降,耐药肺结核病人血清中sCD26水平含量明显上调,二者成负相关,通过检测肺结核患者外周血CD26~+CD4~+T细胞百分含量与血清中sCD26水平能了解肺结核疾病的进程。
Objective To investigate the changes of the percentage of CD26 ~ + CD4 ~ + T cells in peripheral blood and serum sCD26 level in patients with pulmonary tuberculosis. Methods 30 healthy subjects (HD), 30 non-drug-resistant tuberculosis patients (S-TB) and 30 multidrug-resistant tuberculosis (MDR-TB) The percentage of CD26 ~ + CD4 ~ + T cells in each group was counted by Beckman flow cytometry (FCM). The level of sCD26 in peripheral blood was detected by ELISA. Results The percentage of CD26 ~ + CD4 ~ + T cells in peripheral blood of MDR-TB patients was (13.63 ± 3.85)%, which was lower than that of S-TB patients (20.17 ± 5.94%) and HD patients (26.55 ± 5.77 ), The difference was statistically significant (F = 45.037, P = 0.000). The serum sCD26 level in MDR-TB group was (617.99 ± 55.57) μg / ml, which was higher than that in healthy control group (410.23 ± 36.21) μg / ml and S-TB group (435.22 ± 38.34) μg / ml, the difference was statistically significant (F = 197.287, P <0.05). Spearman correlation analysis showed that there was a negative correlation between the percentage of CD26 + CD4 + T cells and the level of serum sCD26 in normal population, S-TB and MDR-TB patients (r = -0.391, P = 0.002). Conclusion The percentage of CD26 ~ + CD4 ~ + T cells in peripheral blood gradually decreases with the progression from non-drug-resistant to drug-resistant tuberculosis, and the level of sCD26 in the serum of drug-resistant pulmonary tuberculosis patients is significantly up-regulated Detection of tuberculosis patients with peripheral blood CD26 ~ + CD4 ~ + T cell percentage and serum sCD26 levels can understand the progress of pulmonary tuberculosis.
