目的应用ROC曲线分析外周血CD4+T细胞表达对肺结核诊断的价值。方法应用流式细胞仪检测肺结核患者95例、非结核肺部疾病患者46例外周血CD4+T细胞亚群的表达。结果 CD4+T细胞的最大曲线下面积(AUC)为0.65,其最大约登指数为24.34,其最佳临界值为328.7×106/L,对应的灵敏度及特异度分别为52.6%和71.7%;且2组低于最佳临界点(328.7×106/L)的患者数差异具有统计学意义(P<0.01),肺结核组明显高于非结核肺部疾病组;而高于临界点时,2组间患者数差异无统计学意义(P>0.05)。结论 CD4+T细胞对肺结核诊断有辅助价值。CD4+T细胞表达水平不同,感染肺结核的几率不同,并且水平越低,肺结核感染的几率越高。
Objective To evaluate the diagnostic value of peripheral blood CD4 + T cells by ROC curve in the diagnosis of pulmonary tuberculosis. Methods The expression of CD4 + T lymphocyte subsets in peripheral blood of 95 patients with tuberculosis and 46 patients with non-tuberculosis pulmonary disease were detected by flow cytometry. Results The maximum area under curve (AUC) of CD4 + T cells was 0.65, and the maximum Youden index was 24.34. The best cutoff value of CD4 + T cells was 328.7 × 106 / L and the corresponding sensitivity and specificity were 52.6% and 71.7%, respectively. (P <0.01). The difference between the two groups was statistically significant (P <0.01), and the TB group was significantly higher than that of the non-tuberculosis lung disease group. Above the critical point, 2 There was no significant difference in the number of patients between groups (P> 0.05). Conclusion CD4 + T cells are valuable for the diagnosis of pulmonary tuberculosis. Different levels of CD4 + T cell expression, the probability of infection of different tuberculosis, and the lower the level, the higher the risk of tuberculosis infection.
