目的:分析心脏病合并心力衰竭的患儿血清脑钠素(BNP)的表达水平,为临床诊治提供参考。方法:选择住院治疗的心力衰竭患儿40例作为观察组,心功能正常的上呼吸道感染住院治疗的患儿20例作为对照组,比较两组患儿血清脑钠素水平;并对观察组患儿作超声心动图记录左室射血分数等心脏功能指标,分析其与血清脑钠素改变的关系。结果:①观察组血清BNP水平明显高于对照组,差异有统计学意义(P<0.05)。②组间单因素方法分析发现,随着心功能降低,LogBNP水平升高,LVEF和LVFS均下降(P<0.05)。③经Spearman相关性分析,观察组血清LogBNP与LVEF、LVFS具有负相关性,血清LogBNP与观察组患儿心功能分级的相关系数明显优于LVEF、LVFS与心功能分级的相关系数,提示LogBNP对于预测心衰的严重程度较LVEF及LVFS更好。结论:BNP是反映心功能受损的敏感指标,与心功能分级具有明显的正相关性,临床检测对心衰患儿诊断和治疗具有重要意义。
Objective: To analyze the expression of serum brain natriuretic peptide (BNP) in children with heart disease complicated with heart failure and provide reference for clinical diagnosis and treatment. Methods: Totally 40 children with HF who underwent hospitalization were selected as the observation group. Twenty children hospitalized with upper respiratory tract infection with normal heart function were selected as the control group. Serum BNP levels were compared between the two groups. Children as echocardiography recorded left ventricular ejection fraction and other cardiac function indicators, analysis and its relationship with changes in serum brain natriuretic peptide. Results: ① The level of serum BNP in the observation group was significantly higher than that in the control group (P <0.05). ② The results of single factor analysis showed that LogBNP level increased, LVEF and LVFS decreased (P <0.05) as cardiac function decreased. ③ Spearman correlation analysis showed that the LogBNP of the observation group was negatively correlated with LVEF and LVFS, and the correlation coefficient between LogBNP and observation group was significantly better than that of LVEF and LVFS and heart function classification, suggesting that LogBNP Predict the severity of heart failure than LVEF and LVFS better. Conclusion: BNP is a sensitive index reflecting the impaired cardiac function and has a significant positive correlation with the grading of cardiac function. Clinical examination is of great significance for the diagnosis and treatment of children with heart failure.
