经颅多普勤(TCD)已广泛应用于脑血管病的研究,但有关病毒性脑炎的TCD研究国内未见报道,经文献检索国外报道均很少.本研究对病毒性脑炎(VE)和TCD检测结果与正常对照组进行了对比研究,目的在于探讨不同病源菌引起的中枢神经系统感染性疾病对脑血流动力学的影响,以及TCD检测在判断病毒性脑炎病情严重程度和临床预后的应用价值.22例病毒性脑炎,30例健康志愿者.应用德国DWL公司P型多普勒超声检查仪,用2MHz探头经颞窗检测双侧大脑前动脉(ACA)、大脑中动脉(MCA)、大脑后动脉(PCA),经枕商检测基底动脉(BA)、椎动脉(VA)的收缩期峰流速(Vs)、舒张期末峰流速(Vd)、平均峰流速(Vm)、脉动指数(PI)、以及频谱形态.根据病人入院时意识状况采用格拉斯哥昏迷量表评分(GCS)分组,对疾病预后按格拉斯哥预后量表(GOS)分级,比较病情严重程度和临床预后与大脑前动脉、大脑中动脉脑血流动力学指标的关系.病毒性脑炎TCD异常改变以颈内动脉系统为主.病毒性脑炎病人大脑中动脉收缩期峰流速越高,病情越严重;TCD检测结果不能判断其预后.TCD是监测中枢神经系统感染性疾病病情变化,动态观察治疗效果,指导临床用药,研究其脑血流动力
