目的 探讨老年冠心病患者血清一氧化氮 ( NO) 、可溶性细胞间黏附分子 -1 ( sICAM-1) 、可溶性血管内皮细胞黏附分子-1 ( sVCAM-1) 、脂联素、生长分化因子-15 ( GDF-15) 的水平变化.方法 选取我院2014 年6 月~2016 年10 月经冠脉造影检测确诊的老年冠心病患者140 例作为冠心病组, 其中稳定性心绞痛 ( SAP) 患者 59 例、不稳定性心绞痛 ( UAP) 患者 48 例、急性心肌梗死( AMI) 患者33 例, 另选40 例健康老年人为对照组.结果 血清NO、脂联素比较, 冠心病组低于对照组(P<0.05); 血清NO、脂联素比较, SAP 组高于 UAP 组和 AMI 组 (P <0.05), UAP 组高于 AMI 组 (P<0.05) ; 血清sICAM-1、 sVCAM-1、 GDF-15 比较, SAP组测定值小于UAP组、 AMI组 ( P <0.05) , UAP组测定值小于AMI组 (P<0.05); 血清NO、脂联素比较, 单支病变患者的测定值大于双支病变组、三支病变组 ( P<0.05) , 双支病变组的测定值大于三支病变组 ( P <0.05) ; 血清 sICAM -1、 sVCAM -1、 GDF-15 的水平比较, 单支病变组测定值小于双支病变组、三支病变组 ( P<0.05) , 双支病变组测定值小于三支病变组 (P<0.05).结论 老年冠心病患者血清 NO、 sICAM -1、 sVCAM -1、脂联素、 GDF-15 水平发生显著变化, 并且与冠心病患者的冠脉病变程度相关.“,”Objective To study the changes of serum nitric oxide (NO) in the elderly patients with coronary heart disease, soluble intercellular adhesion molecule-1 (sICAM-1), soluble vascular cell adhesion molecule-1 (sVCAM-1), adiponectin, growth differentiation factor-15 (GDF-15) levels. Methods In our hospital from June in 2014 to October in 2016 140 elderly pa-tients with coronary heart disease diagnosed by coronary angiography were as the coronary heart disease group, among them 59 patients had stable angina pectoris (SAP group), 48 patients had unstable angina pectoris ( UAP group), 33 patients had acute myocardial infarction (AMI group). 40 healthy elderly were as the control group. Results Levels of NO and adiponectin in coronary heart dis-ease group were lower than those in the control group (P<0.05). Levels of sICAM-1, sVCAM-1 and GDF-15 in coronary heart disease group were significantly higher than those in the control group (P<0.05). Levels of NO and adiponectin in SAP group were higher than those in UAP group and AMI group (P<0.05). Levels of NO and adiponectin were higher in group UAP than those in AMI group (P<0.05). Levels of sICAM-1, sVCAM-1 and GDF-15 in SAP group were significantly lower than those in UAP group and AMI group (P<0.05). Levels of sICAM-1, sVCAM-1 and GDF-15 were significantly lower in UAP group than those in AMI group (P<0.05). Levels of NO and adiponectin in the patients with single vessel lesion were higher than those with double vessels disease and three vessels disease (P<0.05). Levels of NO and adiponectin in double vessel lesion group were higher than those in three vessels lesion group (P<0.05). Levels of sICAM-1, sVCAM-1 and GDF-15 in single vessel lesion group were significantly lower than those in double vessels disease group and three vessels disease group (P<0.05). Levels of sICAM-1, sV-CAM-1 and GDF-15 in double vessels disease group were significantly lower than those in three vessels disease group (P<0.05). Conclusion The changes of serum NO, sICAM-1, sVCAM-1, adiponectin and GDF-15 levels in the elderly patients with coro-nary heart disease were significantly correlated with the severity of coronary artery disease.
