[目的]研究骶骨脊索瘤中CD40、PCNA、CD31的表达水平及其与脊索瘤复发的相关性。[方法]采用EnVi-sionTM免疫组化技术检测28例骶骨脊索瘤组织、9例正常胚胎脊索组织中的CD40和PCNA的表达,并计数CD31标记的肿瘤组织微血管密度(MVD)。[结果]①CD40、PCNA在骶骨脊索瘤中的强阳性表达率分别为66.1%、64.3%,明显高于正常胚胎脊索组织。两者之间的差异有统计学意义(P<0.05);骶骨脊索瘤中CD31标记的微血管密度(24.26±18.68)/mm2,正常胚胎脊索组织微血管密度值为(4.63±2.19)/mm2;差异有统计学意义(P<0.05);②28例骶骨脊索瘤,其中复发组14例,非复发组14例,两组间CD40阳性表达率分别为14.29%、85.71%;PCNA阳性表达率分别为92.86%、50.00%;CD31标记的MVD值分别为(30.66±23.75)/mm2、(17.88±8.52)/mm2;均有统计学差异(P<0.05);③CD40、PCNA表达及CD31标记的MVD值与患者的年龄、性别、肿瘤大小无统计学相关性;④PCNA阳性表达的骶骨脊索瘤组织中MVD值为(28.92±20.27)/mm2,明显高于PCNA阴性组(12.64±3.70)/mm2,差异有统计学意义(P<0.05)。CD40阴性表达的骶骨脊索瘤组织中MVD值为(31.68±22.81)/mm2,明显高于CD40阳性表达组(16.86±9.28)/mm2,差异有统计学意义(P<0.05)。[结论]骶骨脊索瘤中CD40、PCNA、CD31表达水平可作为判断病情进展和肿瘤复发的指标。
[Objective] To study the expression of CD40, PCNA and CD31 in sacral chordoma and its correlation with chordoma recurrence. [Method] EnVi-sionTM immunohistochemical method was used to detect the expression of CD40 and PCNA in 28 chordomas and 9 normal adult chordomas. The CD31-labeled microvessel density (MVD) was counted. [Results] ① The strong positive rates of CD40 and PCNA in sacral chordoma were 66.1% and 64.3%, respectively, which were significantly higher than those in normal embryonic chorda. The difference between the two groups was statistically significant (P <0.05). The CD31-labeled microvessel density (24.26 ± 18.68) / mm2 in sacral chordomas and (4.63 ± 2.19) / mm2 in normal embryonic chordomas were significantly different between the two groups (P <0.05). ② In 28 sacral chordomas, 14 cases were recurrence group and 14 cases were non-recurrence group. The positive rates of CD40 expression in the two groups were 14.29% and 85.71%, respectively. The positive expression rates of PCNA were 92.86 % And 50.00%, respectively. The MVD values of CD31 and CD31 were (30.66 ± 23.75) / mm2 and (17.88 ± 8.52) / mm2, respectively (all P <0.05) The age, gender and tumor size of the patients had no statistical significance. (4) The MVD value of PCNA positive was (28.92 ± 20.27) / mm2, which was significantly higher than that of PCNA negative group (12.64 ± 3.70) / mm2 Statistical significance (P <0.05). MVD in CD40 negative expression sacral chordoma was (31.68 ± 22.81) / mm2, which was significantly higher than that in CD40 positive group (16.86 ± 9.28) / mm2, the difference was statistically significant (P <0.05). [Conclusion] The expression of CD40, PCNA and CD31 in sacral chordoma can be used as an index to judge the progression of disease and tumor recurrence.
