目的:分析比较小血管瘤(SHHE)和小肝癌(SHCC)在螺旋CT多期扫描中的表现,以探讨螺旋CT多期扫描在两者鉴别诊断中的作用和限度。材料与方法:73例患者(38例SHHE,35例SHCC)行平扫和增强扫描。开始注射造影剂后30秒行第一期扫描,尔后在70秒、4分钟行第二期和第三期扫描。观察和比较病灶的强化方式和特征。结果:SHHE组和SHCC组各发现病灶42个,64.29%(27/42)的SHHE具典型表现,如边缘强化明显逐渐向中心填充,30.95%(13/42)两期均为高密度,4.76%(2/42)两期均为低密度,而在第三期扫描中96.67%的SHHE为高密度和等密度。59.52%(25/42)的SHCC具典型表现,如第一期呈高密度,第二期为低密度,23.81%(10/42)第一期为高密度,第二期为等密度,4.76%(2/42)两期均为低密度。第三期扫描中85.71%的SHCC为低密度。结论:根据SHHE和SHCC在螺旋CT双期和或多期扫描中的强化表现,大多数病灶可明确诊断,少数不典型病例尚需做MR以进一步明确。
OBJECTIVE: To analyze the performance of multiple small hemangiomas (SHHE) and small hepatocellular carcinoma (SHCC) in spiral CT multi-phase scans to investigate the role and limits of multi-phase spiral CT in differential diagnosis. Materials and Methods: Plain and enhanced scans were performed in 73 patients (38 SHHE, 35 SHCC). The first phase scan was performed 30 seconds after the start of injection of the contrast agent, and then the second and third phase scans were performed at 70 seconds and 4 minutes. Observe and compare the enhancement methods and characteristics of the lesions. RESULTS: Forty-two lesions were found in SHHE group and SHCC group, and 64.29% (27/42) of the SHHE had typical performances. For example, edge enhancement markedly filled the center, and 30.95% (13/42) of the two phases were both. High-density, 4.76% (2/42) of both phases were low-density, while in the third-phase scan, 96.67% of SHHE were high-density and iso-density. 59.52% (25/42) of the SHCC had typical performances, such as high density in the first phase, low density in the second phase, high density in the first phase of 23.81% (10/42), and the second phase was For isodensity, 4.76% (2/42) of both phases are low density. 85.71% of SHCC in the third scan was low density. CONCLUSIONS: Based on the enhanced performance of SHHE and SHCC in dual-phase and multi-phase scans of spiral CT, most lesions can be clearly diagnosed, and a small number of atypical cases still require MR for further clarification.
