目的分析二维及三维产前超声诊断在胎儿半椎体畸形筛查中的应用价值。方法选取在医院进行产前诊断并确诊为半椎体畸形的胎儿22例,22例孕妇均应用Phlips i U22、GE Voluson E8三维超声成像仪行二维超声检查和三维超声检查。比较二维超声检查与二维超声联合三维超声的诊断结果。结果 22例孕妇均在确诊为胎儿半椎体畸形后实施了引产,引产后的影像学检查证实22例胎儿中胸椎畸形16例,腰椎畸形4例,骶尾椎畸形2例;单发半椎体畸形18例;胸腰段多发半椎体畸形4例,其中合并椎体融合2例。产前二维超声联合三维超声诊断的胎儿半椎体畸形检出率显著高于二维超声诊断,差异具有统计学意义(P<0.05)。产前二维超声与三维超声准确定位椎体率相当,差异无统计学意义(P>0.05)。产前二维超声联合三维超声检查所确定的胎儿椎体病变类型准确率显著高于二维超声诊断,差异有统计学意义(P<0.05)。结论胎儿半椎体畸形产前二维超声检查和三维超声检查具有典型性表现,产前二维超声联合三维超声检查能显著提高胎儿半椎体畸形的检出率,对合理实施产前处理,具有重要的临床意义。
Objective To analyze the value of two-dimensional and three-dimensional prenatal ultrasound in the screening of fetal hemivertebra deformity. Methods Twenty-two fetuses with prenatal diagnosis and hemivertebra deformity diagnosed in the hospital were selected. Twenty-two pregnant women were examined by two-dimensional ultrasonography and three-dimensional ultrasonography with Phlips i U22 and GE Voluson E8 three-dimensional ultrasound images. To compare the diagnostic results of two-dimensional ultrasound and two-dimensional ultrasound combined with three-dimensional ultrasound. Results Twenty-two cases of pregnant women were diagnosed as fetus hemivertebra deformity after induction of labor, imaging after abortion confirmed in 22 cases of fetal thoracic deformity in 16 cases, 4 cases of lumbar deformity, 2 cases of sacrococcygeal malformations; single hemivertebra 18 cases of body deformity; 4 cases of thoracolumbar multiple hemivertebra deformities, including 2 cases of vertebral body fusion. Prenatal two-dimensional ultrasound combined with three-dimensional ultrasound diagnosis of fetal hemivertebra deformity was significantly higher than the two-dimensional ultrasound diagnosis, the difference was statistically significant (P <0.05). Prenatal two-dimensional ultrasound and three-dimensional ultrasound accurate positioning of vertebral body quite, the difference was not statistically significant (P> 0.05). Prenatal two-dimensional ultrasound combined with three-dimensional ultrasound examination of fetal vertebral lesions confirmed by the type of accuracy was significantly higher than the two-dimensional ultrasound, the difference was statistically significant (P <0.05). Conclusion Prenatal two-dimensional ultrasound examination and three-dimensional ultrasound examination of the hemivertebra deformity of the fetus has typical performance. Prenatal two-dimensional ultrasound combined with three-dimensional ultrasound can significantly improve the detection rate of fetal hemivertebra deformity, and prenatal treatment, Has important clinical significance.
