[目的]探讨正常宫颈组织、慢性宫颈炎、CINⅠ~Ⅱ、CINIII和宫颈浸润性鳞癌组织中COX-2(环氧合酶-2)表达及意义。[方法]采用免疫组化Envision法检测正常宫颈组织15例、慢性宫颈炎23例、CINⅠ~Ⅱ25例、CINIII26例及宫颈癌组织42例中COX-2蛋白质的表达。[结果]COX-2蛋白的表达阳性率随着宫颈病变的加重而增高。正常宫颈组织中的COX-2不表达,在慢性宫颈炎、CINⅠ~Ⅱ、CINIII和浸润性宫颈癌的阳性表达率分别为56.5%、52.0%、73.1%和90.5%,正常宫颈组织与各组织COX-2的表达差异有统计学意义(P﹤0.01)。COX-2表达在慢性宫颈炎与CINⅠ~Ⅱ和CINIII差异无统计学意义(P﹥0.05);与浸润性宫颈癌之间差异有统计学意义(χ2=8.229,P﹤0.01)。COX-2表达在CINⅠ~Ⅱ与浸润性宫颈癌(χ2=12.763,P﹤0.01)差异有统计学意义,CINIII与浸润性宫颈癌之间的(χ2=2.417,P﹥0.05)两组之间差异无统计学意义。[结论]COX-2蛋白的表达随着宫颈病变的加重而阳性率增高,其在慢性宫颈炎中的异常表达,对宫颈癌的早期诊断有一定价值。
[Objective] To investigate the expression and significance of cyclooxygenase-2 (COX-2) in normal cervical tissue, chronic cervicitis, CINⅠ-Ⅱ, CINIII and invasive cervical squamous cell carcinoma. [Method] Envision method was used to detect the expression of COX-2 protein in 15 cases of normal cervical tissue, 23 cases of chronic cervicitis, 25 cases of CINⅠ-Ⅱ, 26 cases of CINIII and 42 cases of cervical cancer. [Results] The positive rate of COX-2 protein expression increased with the increase of cervical lesions. The positive expression rate of COX-2 in normal cervical tissue was 56.5%, 52.0%, 73.1% and 90.5% in chronic cervicitis, CINⅠ-Ⅱ, CINIII and invasive cervical cancer respectively. The difference of COX-2 expression was statistically significant (P <0.01). COX-2 expression in chronic cervicitis and CIN Ⅰ ~ Ⅱ and CINIII no significant difference (P> 0.05); and invasive cervical cancer was significantly different (χ2 = 8.229, P <0.01). COX-2 expression was significantly different between invasive cervical cancer (χ2 = 12.763, P <0.01) and CINIII with invasive cervical cancer (χ2 = 2.417, P> 0.05) The difference was not statistically significant. [Conclusion] The positive rate of COX-2 protein expression increases with the increase of cervical lesions. The abnormal expression of COX-2 protein in chronic cervicitis has some value in the early diagnosis of cervical cancer.
