目的:探讨无脾综合征伴双侧气管性支气管应用纤维支气管镜结合64排螺旋计算机断层扫描(CT)检查的临床诊断价值。方法:抽取2014年6月至2016年6月在肇庆市第二人民医院就诊的10例无脾综合征伴双侧气管性支气管患儿进行临床研究,所有患儿均接受纤维支气管镜结合64排螺旋CT检查,分析检查结果。结果:经内镜和多排螺旋CT(MSCT)检查确诊为双侧气管性支气管8例,另外还有两例患儿在经过三维气道重建和胸部CT的联合检查后,发现是右侧气管性支气管,同时还伴有右上肺不张的情况,在经过支气管检查后确诊为双侧气管性支气管。所有患儿均确诊为无脾综合征。结论:无脾综合征伴双侧气管性支气管应用纤维支气管镜结合64排螺旋CT检查具有较高的确诊率,对复杂心血管畸形纠治具有重要的指导作用。
Objective: To investigate the value of bronchoscopy combined with 64-slice spiral computed tomography (CT) in the diagnosis of spleen-free syndrome with bilateral tracheal bronchi. Methods: From June 2014 to June 2016, 10 cases of children with spleen-free syndrome and bilateral tracheal bronchitis treated in the Second People’s Hospital of Zhaoqing City were selected for clinical study. All children underwent bronchoscopy with 64 rows Spiral CT examination, analysis of test results. Results: Eight cases of bilateral tracheal bronchi were diagnosed by endoscopy and MSCT. In addition, two cases were found to be the right trachea after three-dimensional airway reconstruction and chest CT examination Bronchial, but also accompanied by right upper lung atelectasis, after bronchoscopy confirmed bilateral tracheal bronchi. All children were diagnosed as spleen-free syndrome. Conclusion: Splenic syndrome with bilateral tracheal bronchoscopy with fiberoptic bronchoscopy combined with 64-slice spiral CT examination has a high rate of diagnosis, the treatment of complex cardiovascular malformations have an important guiding role.
