目的:探讨孤立性肺结核瘤(SPT)的CT表现。方法:回顾性分析26例经病理确诊的SPT患者的CT资料,所有患者均做了胸部CT平扫,其中15例加做增强扫描。结果:26个病灶中,左右肺各13个病灶,位于上肺尖后段及下肺背段共18个,其余部位8个,病灶长径约3~54 mm,其中正确诊断为肺结核瘤7例,占27%,误诊为周围型肺癌15例,占58%,误诊为炎性假瘤3例,误诊为其他病变1例。结论:SPT容易误诊为肺癌等病变,CT引导下经皮肺穿刺活检是确诊的有效方法。
Objective: To investigate CT findings of solitary pulmonary tuberculosis (SPT). Methods: The CT data of 26 SPT patients diagnosed by pathology were retrospectively analyzed. All patients underwent CT scan of thoracic spine. Fifteen patients underwent enhanced CT scan. Results: Of the 26 lesions, there were 13 lesions in each of the left and right lungs, located in the posterior segment of the apical region and in the posterior segment of the lower lung with 8 in the remaining region. The long diameter of the lesions ranged from 3 to 54 mm, of which the correct diagnosis was tuberculosis 7 Cases, accounting for 27%, misdiagnosed as peripheral lung cancer in 15 cases, accounting for 58%, misdiagnosed as inflammatory pseudotumor in 3 cases, misdiagnosed as other lesions in 1 case. Conclusion: SPT is easily misdiagnosed as lung cancer and other lesions, CT-guided percutaneous lung biopsy is an effective diagnosis method.
