目的系统评价降钙素原(PCT)对结核性胸腔积液的诊断价值。方法计算机检索CNKI、WangFang Data、VIP、CBM、PubMed、h e Cochrane Library和EMbase数据库,纳入PCT与金标准(结核菌培养结果阳性)比较诊断结核性胸腔积液的研究,检索时限均为从建库至2013年8月。由2名研究者按照纳入与排除标准独立筛选文献、提取资料和评价文献质量后,采用MetaDisc 1.4软件进行Meta分析。结果最终纳入8个研究。Meta分析结果显示:SPE合并=0.76[95%CI(0.70,0.81)]、SEN合并=0.63[95%CI(0.58,0.68]、+LR=2.72[95%CI(1.48,5.02)]、–LR=0.49[95%CI(0.29,0.82)]、DOR合并=5.77[95%CI(1.89,17.58)]、SROC曲线下面积(AUC)为0.79。结论 PCT对结核性胸腔积液诊断具有一定的临床价值,可用于结核性胸腔积液的鉴别诊断。
Objective To evaluate the value of procalcitonin (PCT) in the diagnosis of tuberculous pleural effusion. Methods The databases of CNKI, WangFang Data, VIP, CBM, PubMed, he Cochrane Library and EMbase were searched by computer and included in the study of PCT and gold standard (positive results of TB culture) in the diagnosis of tuberculous pleural effusion. Until August 2013. Two researchers independently screened the literature according to inclusion and exclusion criteria, extracted data and evaluated the quality of the literature. Meta-analysis was performed using MetaDisc 1.4 software. The results eventually included 8 studies. Meta analysis showed that SPE incorporation was 0.76 [95% CI (0.70,0.81)], SEN incorporation was 0.63 [95% CI (0.58,0.68], + LR = 2.72 [95% CI 1.48,5.02] The area under the curve of SROC (AUC) was 0.79. Conclusions The PCT has a certain value in the diagnosis of tuberculous pleural effusion (LR = 0.49 [95% CI 0.29,0.82] The clinical value of tuberculosis can be used for the differential diagnosis of pleural effusion.
