目的:分析妊娠晚期未足月胎膜早破的临床特点。方法:选取妊娠晚期未足月胎膜早破产妇133例为研究对象,给予常规的治疗措施。分析患者发生胎膜早破的原因,观察患者自胎膜早破到分娩的时间、分娩方式和新生儿的出生情况。结果:胎膜早破的发生原因最常见的是阴道炎,占36.84%;其次是胎位不正,占12.03%;原因不明者占38.05%。70.68%的产妇在发生胎膜早破48 h后进行分娩;在胎膜早破后24~48 h和24 h内分娩的产妇分别有16.54%和12.78%。55.64%的产妇分娩方式为自然分娩,24.81%为剖宫产,臀位助产、胎吸助产、产钳助产分别占12.03%、6.02%和1.50%。死亡新生儿11例,死亡率为8.15%,其中7例死于分娩后的重度窒息,4例死于肺透明膜病和肺部感染。结论:生殖道感染、胎位不正等因素可导致胎膜早破,临床上应重视胎膜早破诱发因素的防治,适当应用糖皮质激素等药物,选择合适的分娩时机和分娩方式,以减少产妇和胎儿并发症,改善新生儿的结局。
Objective: To analyze the clinical features of premature rupture of membranes in the third trimester of pregnancy. Methods: 133 cases of premature rupture of membranes of preterm third trimester of pregnancy were selected as the research object, given conventional treatment. Analysis of the causes of premature rupture of membranes in patients with premature rupture of membranes to the delivery time, delivery mode and the birth of newborns. Results: Premature rupture of membranes was the most common cause of vaginitis, accounting for 36.84%; followed by malposition of the fetus, accounting for 12.03%; unexplained 38.05%. 70.68% of the mothers gave birth 48 hours after the premature rupture of membranes, and 16.54% and 12.78% of the mothers gave birth within 24-48 hours and 24 hours after the premature rupture of membranes. 55.64% of the delivery methods were spontaneous delivery, 24.81% were cesarean section, breech delivery, fetus abortion and forceps delivery were 12.03%, 6.02% and 1.50% respectively. Eleven newborns died with a mortality rate of 8.15%, of whom 7 died of severe asphyxia after delivery and 4 died of hyaline membrane disease and lung infection. Conclusions: Factors such as genital tract infection and abnormal fetal position can lead to premature rupture of membranes. Clinically, attention should be paid to prevention and treatment of premature rupture of membranes. Appropriate application of glucocorticoid and other drugs should be given to choose appropriate timing of delivery and mode of delivery to reduce maternal And fetal complications, improve the outcome of newborns.
