目的 探讨异丙酚对学龄前儿童静脉滴注氯胺酮药代动力学的影响。方法 16例3.5~6 岁行择期手术的学龄前儿童随机分为两组:Ⅰ组为氯胺酮组,Ⅱ组为氯胺酮+ 异丙酚组。Ⅰ组采用异氟醚吸入诱导,Ⅱ组采用异丙酚诱导。气管插管后两组均静脉滴注氯胺酮3m g·kg- 1 ·h- 1(0.05% ),同时吸入异氟醚行麻醉维持。应用反向高效液相色谱法测定氯胺酮血药浓度,采用中国药理学会编辑的3P97软件计算药代动力学参数。结果 两组房室模型均拟合为二房室开放模型。药代动力学参数:Ⅰ组T1/2 α= (0.15±0.04)h,T1/2 β= (2.84±0.44)h,Cl= (0.34±0.06)L·kg- 1 ·h- 1,Vd= (0.66±0.29)L·kg- 1 ;Ⅱ组T1/2α= (0.14±0.06)h,T1/2β= (2.67±0.25)h,Cl= (0.33±0.08)L·kg- 1 ·h- 1 ,Vd= (0.67±0.22)L·kg- 1 。各药代动力学参数两组间无显著性差异。结论 异丙酚不影响氯胺酮在学龄前儿童体内的代谢。
Objective To investigate the effect of propofol on the pharmacokinetics of ketamine in preschool children. Methods Sixteen preschoolers aged 3.5-6 years undergoing elective surgery were randomly divided into two groups: group Ⅰ was ketamine, group Ⅱ was ketamine + propofol. Group Ⅰ was induced by inhalation of isoflurane, and group Ⅱ was induced by propofol. After intubation intratracheal intratracheal intratracheal instillation of ketamine (3 mg · kg-1 · h-1, 0.05%), inhalation of isoflurane anesthesia was maintained. The plasma concentration of ketamine was determined by reverse-phase high performance liquid chromatography. Pharmacokinetic parameters were calculated using 3P97 software edited by the Chinese Pharmacological Society. Results The two atrioventricular models were fitted as two-compartment open model. Pharmacokinetic parameters: Group Ⅰ T1 / 2 α = (0.15 ± 0.04) h, T1 / 2 β = (2.84 ± 0.44) h, Cl = ) L · kg-1 · h-1, Vd = (0.66 ± 0.29) L · kg-1; Group Ⅱ T1 / 2α = (0.14 ± 0.06) h, T1 / 2β = 2.67 ± 0.25) h, Cl = (0.33 ± 0.08) L · kg-1 · h-1, Vd = (0.67 ± 0.22) L · kg-1. The pharmacokinetic parameters of the two groups showed no significant difference. Conclusion Propofol does not affect the metabolism of ketamine in preschool children.
