目的研究电子纤维支气管镜肺泡灌洗联合雾化吸入治疗慢性阻塞性肺疾病(COPD)肺心病合并肺部感染的疗效。方法将COPD肺源性心脏病合并肺部感染患者分两组,常规治疗组给予常规氧疗、抗感染、解痉、对症治疗;综合治疗组在常规治疗基础上,采用电子纤维支气管镜肺泡灌洗联合雾化吸入治疗。治疗4 d后,对比两组患者胸片变化和动脉血气指标、肺功能改善情况等指标变化。结果综合治疗组较常规治疗组患者肺炎吸收时间明显缩短,患者血氧饱和度水平回升明显,低氧血症和二氧化碳潴留情况纠正时间明显缩短。综合治疗组患者肺功能改善较常规治疗组明显。结论电子纤维支气管镜肺泡灌洗联合雾化吸入治疗COPD肺源性心脏病合并肺部感染,使患者肺功能改善明显,肺炎吸收时间明显缩短,减少医疗总费用,减轻患者痛苦,减少住院时间,有较好的社会效益。
Objective To study the curative effect of electronic bronchofibroscopy and bronchoalveolar lavage combined with inhalation in the treatment of pulmonary heart disease complicated with chronic obstructive pulmonary disease (COPD). Methods COPD patients with pulmonary heart disease complicated with pulmonary infection were divided into two groups. Patients in the conventional treatment group were given routine oxygen therapy, anti-infective, antispasmodic and symptomatic treatment. On the basis of routine treatment, the patients in the comprehensive treatment group were treated with electron bronchoscopy Washing combined with inhalation therapy. After 4 days of treatment, changes of chest radiograph, arterial blood gas index and improvement of lung function were compared between the two groups. Results Compared with the conventional treatment group, the pneumoconiosis absorption time was significantly shortened in patients in the comprehensive treatment group, the blood oxygen saturation level rose significantly, and the correction time of hypoxemia and carbon dioxide retention was significantly shortened. The lung function of patients in the comprehensive treatment group was better than that in the conventional treatment group. Conclusions Electronic bronchofibersophageal alveolar lavage combined with atomized inhalation in the treatment of pulmonary heart disease complicated with pulmonary infection in patients with COPD can improve lung function, shorten the time of pneumonia absorption, reduce the total cost of medical treatment, reduce the suffering of patients and reduce the length of hospital stay, Have better social benefits.
