目的检测抗苗勒管激素(AMH)在卵巢早衰(POF)、多囊卵巢综合征(PCOS)患者以及正常健康女性血清中的变化水平并探讨其在产前筛查方面的临床意义与价值。方法选取本院POF患者(POF组)39例,PCOS患者(PCOS组)42例,并选取同期接受健康体检且月经周期正常的女性45例作为对照组。测定上述3组女性血清中AMH、卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)及睾酮(T)的水平。结果 PCOS组AMH、E2和T水平高于POF组和对照组,而对照组AMH和E2水平高于POF组,差异有统计学意义(P<0.05);POF组FSH、LH高于PCOS组和对照组,而PCOS组LH高于对照组,差异有统计学意义(P<0.05),PCOS组与对照组FSH相比无统计学差异(P>0.05)。结论 AMH在不同卵巢储备功能的PCOS、POF患者以及正常健康女性中表达水平不同,因而联合检测血清中AMH与性激素水平在诊断PCOS和POF以及产前筛查方面具有一定的临床应用价值。
Objective To detect the level of anti-Mullerian hormone (AMH) in sera of patients with premature ovarian failure (POF), polycystic ovary syndrome (PCOS) and normal healthy women and to explore its clinical significance and value in prenatal screening. Methods 39 cases of POF patients (POF group) and 42 cases of PCOS patients (POOS group) were selected. 45 women with normal menstrual cycle during the same period were selected as the control group. The levels of AMH, follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2) and testosterone (T) in the above three groups of women were measured. Results The levels of AMH, E2 and T in PCOS group were higher than those in POF group and control group, while the levels of AMH and E2 in control group were higher than those in POF group (P <0.05) (P <0.05). There was no significant difference between the PCOS group and the control group (P> 0.05). Conclusion The expression of AMH is different between PCOS and POF patients with different ovarian reserve functions and normal healthy women. Therefore, the combined detection of serum AMH and sex hormone levels has some clinical value in the diagnosis of PCOS, POF and prenatal screening.
