目的探讨出生3天内新生儿惊厥的电解质变化。方法收集1993~1994年我院新生儿室收治的出生3天内新生儿惊厥患儿65例。抽取静脉血,采用COBASMIRA生化分析仪进行电解质测定,并分别研究其胎龄、出生体重、母妊娠史和生产史、病因、CT扫描脑损伤分度与血电解质紊乱的关系及治疗效果。统计学处理采用卡方检验。结果低钠血症43.08%,低钙血症55.39%,高钾血症44.6%,酸中毒96.92%,CT扫描示重度脑损伤病例血钠,血钙下降者明显多于轻度病例(P<0.05)。结论出生3天内新生儿惊厥易于发生电解质紊乱,尤其是低钠血症,低钙血症,其发生与原发病密切相关。
Objective To investigate the changes of electrolytes in newborn infants within 3 days of birth. Methods A total of 65 neonates with convulsions within 3 days after birth were collected from the neonatal room in our hospital from 1993 to 1994. Venous blood samples were collected for determination of electrolytes by COBASMIRA biochemical analyzer. The relationship between gestational age, birth weight, mothers’ pregnancy history and production history, etiology, CT scan grade and electrolyte imbalance were studied. Statistical analysis using chi-square test. Results 43.08% of hyponatremia, 55.39% of hypocalcemia, 44.6% of hyperkalemia and 96.92% of acidosis. CT scan showed that serum sodium and serum calcium decreased significantly in severe brain injury cases than in mild cases P <0.05). Conclusions Neonatal convulsions within 3 days of birth are prone to electrolyte imbalance, especially hyponatremia and hypocalcemia. The occurrence of eclampsia is closely related to the primary disease.
