目的分析高血压合并2型糖尿病患者踝臂指数(ABI)和脉搏波速度(PWV)的影响因素及两者之间的关系。方法入选2015年5月—2016年3月期间承德医学院附属医院心脏内科就诊的原发性高血压患者60例(A组),内分泌科住院的2型糖尿病患者60例(B组),高血压合并2型糖尿病患者120例(C组),健康者45例(对照组),分别测量各组研究对象ABI、PWV、C反应蛋白(CRP)、血压(BP)、空腹血糖(FBG)及血生化指标等相关参数。结果四组之间年龄、性别、体质量指数(BMI)等一般资料比较差异无统计学意义(P>0.05);A、B、C组的PWV值明显高于对照组(P<0.05);B、C组的ABI值明显低于对照组(P<0.05);C组PWV值明显高于其余三组(P<0.05);C组ABI值明显低于其余三组(P<0.05);A组ABI值与对照组比较差异无统计学意义(P>0.05)。ABI的危险因素为FBG、CRP(P<0.05)。PWV与年龄、收缩压(SBP)和舒张压(DBP)呈明显的正相关(P<0.05)。直线回归分析表明PWV和ABI有直线关系。结论 ABI和PWV联合检测可用于评估高血压合并2型糖尿病患者动脉硬化程度,为临床早期干预治疗提供较好的应用价值。
Objective To analyze the influencing factors of ankle brachial index (ABI) and pulse wave velocity (PWV) in patients with type 2 diabetes mellitus (Hypertension) and their relationship. Methods Sixty patients with primary hypertension (group A) and 60 patients with type 2 diabetes (group B) admitted to Department of Endocrinology, Affiliated Hospital of Chengde Medical College from May 2015 to March 2016 were selected as high 120 patients with type 2 diabetes mellitus (group C) and 45 healthy people (control group) were included in this study. ABI, PWV, CRP, BP, FBG, Blood biochemical parameters and other related parameters. Results There was no significant difference in the general data such as age, sex, body mass index (BMI) between the four groups (P> 0.05). The PWV of group A, B and C were significantly higher than that of the control group (P <0.05). The ABI of group B and C were significantly lower than that of the control group (P <0.05). The PWV of group C was significantly higher than that of the other three groups (P <0.05). The ABI of group C was significantly lower than that of the other three groups (P <0.05). There was no significant difference in ABI value between A group and control group (P> 0.05). The risk factors for ABI were FBG and CRP (P <0.05). There was a significant positive correlation between PWV and age, SBP and DBP (P <0.05). Linear regression analysis showed that there was a linear relationship between PWV and ABI. Conclusion The combination of ABI and PWV can be used to evaluate the degree of atherosclerosis in type 2 diabetes mellitus patients with hypertension and provide a good clinical value for early intervention.
