患者,女,27岁,因面部红斑四肢关节酸痛、面部浮肿、低热1年,症状加重伴有双下肢水肿、胸闷、脱发、皮肤红斑半年而入院。患者曾诊断为全身性红斑狼疮(SLE)。经用激素、硫唑嘌呤等药物治疗,病情缓解。近半年来,出现脱发,全身浮肿、皮肤红班、呼吸困难、四肢乏力和视力进行性下降,以SLE再次住院。其胞妹也患有SLE。体格检查:体温37.8℃,呼吸24次,脉搏100次/分,血压20/11kPa,神志清楚,面部眼睑肿胀,头发稀疏,
