目的了解幽门螺杆菌(Hp)感染对延髓麻痹患者肺部感染的发病率及病原菌构成比的影响,以降低肺部感染率。方法采用回顾性调查的方法,对2010年1月-2013年12月1 225例吞咽功能障碍患者进行Hp感染、肺部感染发病率及病原菌进行分析,所有入院24h内患者行吞咽功能障碍评估及14 C呼气试验,数据应用SPSS 19.0软件进行统计分析。结果 512例Hp感染阳性延髓麻痹患者肺部感染27例,感染率5.27%,轻、中、重度吞咽功能障碍的感染发病率分别为2.90%、5.24%、9.09%;713例Hp感染阴性的延髓麻痹患者肺部感染68例,感染率9.54%,轻、中、重度吞咽功能障碍的感染发病率分别为8.65%、9.67%、10.37%;Hp感染阳性组检出革兰阴性菌、革兰阳性菌、真菌分别占60.53%、36.84%、2.63%,Hp感染阴性组分别占66.67%、30.95%、2.38%。结论 Hp感染能减少延髓麻痹患者肺部感染的发病率,特别是轻、中度吞咽功能障碍的患者,Hp感染并不影响肺部感染的病原菌比例。
Objective To understand the influence of Helicobacter pylori (Hp) infection on the incidence of pulmonary infection and the composition ratio of pathogenic bacteria in patients with bulbar paralysis, in order to reduce the rate of pulmonary infection. Methods A retrospective study was conducted to analyze the incidence of Hp infection, the incidence of pulmonary infection and pathogenic bacteria in 1 225 swallowing dysfunction patients from January 2010 to December 2013 and to evaluate the swallowing dysfunction in all patients admitted within 24 h 14 C breath test, the data using SPSS 19.0 software for statistical analysis. Results The infection rates of lung infection in 512 Hp positive patients with bulbar paralysis were 27 (infection rate 5.27%, 2.90%, 5.24%, 9.09% respectively), 713 patients with negative Hp infection 68 cases of pulmonary infection in paralyzed patients, the infection rate was 9.54%, the incidence of mild, moderate and severe swallowing dysfunction were 8.65%, 9.67%, 10.37% respectively; Gram-negative bacteria were detected in Hp infection positive group, The fungi and fungi accounted for 60.53%, 36.84% and 2.63% of the total, respectively. The positive rate of Hp infection was 66.67%, 30.95% and 2.38% respectively. Conclusions Hp infection can reduce the incidence of pulmonary infection in patients with bulbar paralysis, especially in patients with mild to moderate swallowing dysfunction. Hp infection does not affect the proportion of pathogenic bacteria in lung infection.
