目的探讨宫颈阴道分泌物中胰岛素样生长因子结合蛋白-1(IGFBP-1)检测微小胎膜早破的临床应用。方法收集许昌市人民医院2013年2月—2014年11月胎膜早破孕妇,尤其是微小高位胎膜早破的孕妇从中任取80例为实验组,胎膜未破的孕妇82例为对照组。均采用IGFBP-1试剂检测和p H试纸两种方法检测,并将检测结果进行对照。结果 IGFBP-1检测胎膜早破的灵敏度为93.75%(75/80),特异度为98.78%(81/82),阳性预测值98.68%(75/76),阴性预测值为94.19%(81/86)。优于目前临床上应用最多的p H试纸法[灵敏度25%(20/80)]。结论阴道分泌物中检测IGFBP-1(胶体金法)可作用快速检测微小胎膜早破的有效方法,以便及时对症处理,避免不良结局的发生。
Objective To investigate the clinical application of insulin-like growth factor binding protein-1 (IGFBP-1) in cervical vaginal secretions for the detection of small premature rupture of membranes. Methods From October 2013 to November 2014, 80 pregnant women with premature rupture of membranes, especially pregnant women with tiny high premature rupture of membranes, were enrolled in this study. Totally 80 pregnant women were selected as the experimental group, and 82 pregnant women without rupture of membranes as control group. Both IGFBP-1 reagent and p H test strips were used to detect the test results. Results The sensitivity of IGFBP-1 in detecting premature rupture of membranes was 93.75% (75/80), the specificity was 98.78% (81/82), the positive predictive value was 98.68% (75/76), the negative predictive value was 94.19% (81 / 86). Better than the current most widely used p H test strip method [sensitivity 25% (20/80)]. Conclusions IGFBP-1 (colloidal gold assay) can be used to detect the early rupture of tiny fetal membranes in vaginal secretions, so that it can be treated symptomatically and avoid the unpleasant consequences.
