经阴道三维子宫输卵管造影对不孕症的诊断价值

目的:探讨经阴道三维子宫输卵管造影(3D-HyCoSy)对不孕症的诊断价值。方法:抽取2018年7月至2020年7月郑州市第七人民医院收治的不孕症患者60例。所有患者均接受3D-HyCoSy检查,并于检查后2个月内进行腹腔镜通染液试验。以腹腔镜通染液试验为基准,评价3D-HyCoSy的诊断符合率及诊断效能。结果:本组共60例患者(120条输卵管),3D-HyCoSy检查结果示输卵管通畅42条,通而不畅38条,阻塞40条;腹腔镜通染液试验结果显示,输卵管通畅43条,通而不畅37条,阻塞40条。经3D-HyCoSy诊断通畅、通而不畅、阻塞的符合率分别为90.70%、91.89%、87.50%,一致性较高(n P>0.05)。将通而不畅、阻塞合计为不通畅,进行二次分析,结果显示,3D-HyCoSy诊断输卵管不畅的灵敏度为96.10%,特异度为90.70%,准确度为94.17%,阳性预测值为94.87%,阴性预测值为92.86%,与腹腔镜通染液试验比较差异未见统计学意义(n P>0.05)。n 结论:3D-HyCoSy诊断输卵管通畅性的价值较高,可作为输卵管通畅性评价的首选方案。“,”Objective:To investigate the diagnostic value of three-dimensional hysterosalpingo-contrast sonography (3D-HyCoSy) for infertility.Methods:A total of 60 cases of infertility patients treated in Zhengzhou Seventh People’s Hospital from July 2018 to July 2020 were selected. All the patients underwent 3D-HyCoSy examination. And laparoscopic staining solution test was performed on enrolled patients within 2 months after examination. The results of laparoscopic staining solution test was used as a benchmark, the diagnostic coincidence rate and diagnostic efficacy of 3D-HyCoSy for infertility were evaluated.Results:A total of 60 patients (120 fallopian tubes) were enrolled into the study. The results of 3D-HyCoSy showed that 42 tubes were unobstructed, 38 tubes were partially obstructed, and 40 tubes were completely obstructed. The results of laparoscopic staining solution test showed that 43 tubes were unobstructed, 37 tubes were partially obstructed, and 40 tubes were completely obstructed. The coincidence rates of 3D-HyCoSy in diagnosis of unobstructed, partially obstructed, and completely obstructed were 90.70%, 91.89%, and 87.50%, respectively, all of which were high (n P>0.05). The partially obstructed and completely obstructed were allocated into the obstructed group, and the second analysis showed that the sensitivity, specificity, accuracy, positive predictive value, and the negative predictive value of 3D-HyCoSy in diagnosing tubal obstructed were 96.10%, 90.70%, 94.17%, 94.87%, and 92.86%, respectively, which were significantly different from those of laparoscopic staining solution test (n P>0.05).n Conclusions:3D-HyCoSy is of high value in diagnosing tubal patency, and it can be used as the first choice for evaluating tubal patency.
