目的:观察激素(地塞米松)预处置对兔油酸型急性呼吸窘迫综合症(ARDS)血清TNF-a、IL-8和IL-10的影响,探讨激素治疗ARDS的理论基础。方法:经兔耳缘静脉注射油酸复制兔ARDS模型,用酶联免疫吸附试验测定血清中TNF-a、IL-8和IL-10水平,结合血气分析检测及肺组织病理学改变进行观察分析。结果:ARDS组兔血清TNF-a、IL-8水平明显高于对照组(P<0.01);IL-10水平明显低于对照组(P<0.01)。地塞米松预处置组血清IL-10水平有上升恢复,高于ARDS组(P<0.01)。地塞米松预处置组血清TNF-a、IL-8水平降低,低于ARDS组(p<0.01)。动脉血气分析及肺组织病理学有相应改变。结论:TNF-a、IL-8和IL-10在兔油酸型ARDS炎症中可能起重要作用,地塞米松可能通过抑制TNF-a、IL-8过度分泌,促进IL-10合成而减轻肺损伤。
Objective: To observe the effect of hormone (dexamethasone) preconditioning on serum TNF-a, IL-8 and IL-10 in acute oral distress syndrome (ARDS) of rabbits and to explore the theoretical basis of hormone therapy for ARDS. Methods: Rabbits were anesthetized with oleic acid for rabbit ARDS model. The levels of TNF-a, IL-8 and IL-10 in serum were detected by enzyme-linked immunosorbent assay (ELISA), combined with blood gas analysis and lung histopathological changes . Results: Serum levels of TNF-a and IL-8 in ARDS group were significantly higher than those in control group (P <0.01). The level of IL-10 in ARDS group was significantly lower than that in control group (P <0.01). Dexamethasone pretreatment serum IL-10 levels increased recovery, higher than the ARDS group (P <0.01). Dexamethasone pretreatment serum TNF-a, IL-8 levels lower than ARDS group (p <0.01). Arterial blood gas analysis and lung histopathology have been changed accordingly. CONCLUSION: TNF-a, IL-8 and IL-10 may play an important role in the inflammation of rabbit oleic acid-induced ARDS. Dexamethasone may reduce lung by inhibiting the secretion of TNF-a and IL-8 and promoting IL-10 synthesis damage.
