目的探讨肿瘤坏死因子-α(TNF-α)基因308位点多态性与尘肺易感关系。方法检索中外数据库于1994年1月~2011年8月公开发表的文献,共检索到关于TNF-α基因308位点多态性与尘肺发病风险相关的病例-对照研究18篇。对文献进行Meta分析,并按尘肺类型和种族进行分层分析,计算合并OR值(95%CI)。结果共纳入文献10篇,累计病例组1408例,对照组1639名;分析显示,携带Arg/Arg、Gln/Aln、Gln/Arg+Arg/Arg基因型个体与携带Gln/Gln基因型个体相比,尘肺发病风险分别高1.89(95%CI:1.10~3.24)、1.53(95%CI:1.25~1.87)、1.56(95%CI:1.28~1.90)倍;分层分析显示,煤工人群中携带Arg/Arg、Gln/Arg、Gln/Arg+Arg/Arg基因型的个体与携带Gln/Gln基因型者相比,尘肺发病风险分别高2.29(95%CI:1.22~4.29)、1.56(95%CI:1.20~2.03)、1.64(95%CI:1.28~2.11)倍;亚洲人群中携带Gln/Arg、Gln/Arg+Arg/Arg基因型个体与携带Gln/Gln基因型者相比,尘肺发病风险分别高1.58(95%CI:1.28~1.95)、1.57(95%CI:1.28~1.94)倍。结论 TNF-α基因308位点Arg/Arg、Gln/Arg、Gln/Arg+Arg/Arg基因型携带者易患尘肺。
Objective To investigate the relationship between TNF-α 308 locus polymorphism and susceptibility to pneumoconiosis. Methods A total of 18 case-control studies on the polymorphisms of TNF-α gene at 308 locus associated with the risk of pneumoconiosis were retrieved from Chinese and foreign databases published from January 1994 to August 2011. Meta-analyzes were performed on the literature and stratified by pneumoconiosis type and ethnicity to calculate the pooled OR (95% CI). Results A total of 10 articles were included in the literature, with a total of 1,408 cases in the control group and 1639 cases in the control group. The results showed that individuals carrying Arg / Arg, Gln / Aln, Gln / Arg + Arg / Arg genotypes compared with individuals carrying the Gln / Gln genotype , And the risk of pneumoconiosis was 1.89 (95% CI: 1.10-3.24), 1.53 (95% CI: 1.25-1.87) and 1.56 (95% CI: 1.28-1.90), respectively.
