目的:探讨原发性三叉神经痛、特发性半面痉挛的病因及发病机理。方法:对比研究4例原发性三叉神经痛、2例特发性半面痉挛、2例症状性三叉神经痛及2例正常三叉神经、面神经根的透射电镜结果,并结合临床综合分析。结果:发现原发性三叉神经痛、特发性半面痉挛的超微结构改变基本相同,与症状性三叉神经痛和正常神经根的超微结构进行对比,证实血管与肿物对神经根的压迫导致了神经超微结构改变。结论:三叉神经痛和半面痉挛的发病机理可能为神经纤维间“短路”和神经核功能紊乱。神经根超微结构改变的严重程度与神经根受压程度有关而与受压部位、病程长短无明显关系
Objective: To investigate the etiology and pathogenesis of primary trigeminal neuralgia and idiopathic hemifacial spasm. Methods: Four cases of primary trigeminal neuralgia, two cases of idiopathic hemifacial spasm, two cases of symptomatic trigeminal neuralgia and two cases of normal trigeminal nerve and facial nerve root were studied by transmission electron microscopy and clinical comprehensive analysis. Results: The findings of primary trigeminal neuralgia, idiopathic hemifacial spasm ultrastructural changes were basically the same, and symptomatic trigeminal neuralgia and normal nerve root ultrastructure were compared to confirm the vascular and tumor compression of the nerve root Leading to changes in the ultrastructure of the nerve. Conclusion: The pathogenesis of trigeminal neuralgia and hemifacial spasm may be “short circuit” between nerve fibers and neuronal dysfunction. The severity of nerve root ultrastructure changes was related to the degree of nerve root compression but not to the compression site and duration of disease
