目的:评估蒙特利尔认知量表(MoCA)对军队老年人群认知功能测试的敏感度,为确定筛选轻度认知功能障碍(MCI)轻度认知损害的分界值进行初步探讨。方法:选择军队离退休干部103例,MCI 36例作为观察组,正常67例作为对照组,回顾性研究两组MoCA和简易精神状态检查量表(MMSE)的测评结果。计算正常与MCI不同分界值时检出MCI的敏感度和特异度。结果:MMSE与MoCA测评总分间差异显著(P<0.05)。当分界值为26分时,MMSE检测MCI的敏感度为16.67%、特异性为100.00%;MoCA检测MCI的敏感度为94.44%、特异性为59.70%;分界值为25分时,MoCA检测MCI的敏感度为91.67%、特异性为70.15%。结论:MoCA较适合用于筛查老年MCI,划分正常与MCI的分界值选择25分较适宜。
Objective: To assess the sensitivity of Montreal Cognitive Scale (MoCA) to the cognitive function tests of the elderly in the military and to discuss the criteria for screening mild cognitive impairment (MCI). Methods: 103 military retired cadres, 36 MCI as observation group and 67 normal control group were selected as control group. The results of MoCA and MMSE were retrospectively studied. The sensitivity and specificity of detecting MCI when calculating the normal and MCI different cut-off values were calculated. Results: There was significant difference between MMSE and MoCA in total score (P <0.05). When the cut-off value was 26, the sensitivity and specificity of MMSE for detecting MCI were 16.67% and 100.00% respectively. The sensitivity and specificity of MoCA for detecting MCI were 94.44% and 59.70% respectively. When the cut-off value was 25, The sensitivity was 91.67% and the specificity was 70.15%. Conclusion: MoCA is more suitable for screening elderly MCI, the division of normal and MCI cut-off value of 25 points is more appropriate choice.
