目的探讨2型糖尿病患者尿微量白蛋白/肌酐比值与认知功能的相关性。方法 136例2型糖尿病患者,根据尿微量白蛋白/肌酐比值分为A(≤30mg/g),B(>30~300mg/g),C(>300mg/g)组,并依据是否合并糖尿病视网膜病变分为视网膜病变组和非视网膜病变组,以认知功能问卷调查方式进行认知功能评定,分析糖尿病患者认知功能同尿微量白蛋白/肌酐比值的相关性。结果 A,B,C组简易智能状态量表总分、听觉词语即刻记忆3个、身体部位延时记忆、指图总耗时、图片补缺、图片误认数、图片延时记忆、图片即刻记忆以及数字广度等比较差异有统计学意义(P<0.05);3组高血压病程、收缩压、C-反应蛋白及胰岛素抵抗指数等比较差异有统计学意义(P<0.05);尿微量白蛋白/肌酐比值及糖尿病视网膜病变与收缩压呈正相关(r=0.322,r=0.231),与胰岛素抵抗指数及伴冠心病呈正相关(r=0.309,r=0.302);微血管病变、是否伴冠心病与配偶健康程度3个自变量对该模型具有统计学意义(P<0.05)。结论糖尿病血管病变与患者尿微量白蛋白/肌酐比值有密切相关性;尿微量白蛋白/肌酐比值与胰岛素抵抗指数呈正相关;2型糖尿病早期有微量白蛋白尿时已发生认知功能变化,应尽早进行预防并治疗。
Objective To investigate the correlation between urinary albumin / creatinine ratio and cognitive function in type 2 diabetic patients. Methods One hundred and sixty - six patients with type 2 diabetes mellitus were divided into A (≤30mg / g), B (> 30 ~ 300mg / g) and C (> 300mg / g) according to the urinary albumin / creatinine ratio Retinopathy was divided into two groups: retinopathy group and non-retinopathy group. Cognitive function was assessed by cognitive function questionnaire, and the correlation between cognitive function and urinary albumin / creatinine ratio was analyzed. Results A, B, C group of simple intelligent state total score, the immediate memory of auditory words 3, the body part of the delay memory, referring to the total picture time, picture fill the missing, picture recognition, picture delay memory, picture instant memory (P0.05) .High blood pressure duration, systolic blood pressure, C-reactive protein and insulin resistance index in the three groups were significantly different (P <0.05); urinary microalbumin / Creatinine ratio and diabetic retinopathy were positively correlated with systolic blood pressure (r = 0.322, r = 0.231), and insulin resistance index and coronary heart disease were positively correlated (r = 0.309, r = 0.302); microvascular disease, Three independent variables of spouse’s health degree had statistical significance (P <0.05). Conclusions There is a close relationship between diabetic microvascular disease and urinary albumin / creatinine ratio. Urinary microalbumin / creatinine ratio is positively correlated with insulin resistance index. Cognitive function changes have occurred in microalbuminuria in early stage of type 2 diabetes. Early prevention and treatment.
