作者于1985年3月至7月用超声波吸引(Sonotec)和CO_2激光联合切除颅内肿瘤23例,其中脑膜瘤17例,胶质瘤5例。前颅凹底低分化癌1例。本组病例皆属部位深或在功能区的肿瘤,近期效果良好,术后反应轻,无1例神经系统损害加重。 我们认为超声吸引最大优点是保留瘤内血管(1mm以上直径者)的同时能破碎和吸除瘤组织,从而减小肿瘤体积,以便在不牵拉正常脑组织情况下切除肿瘤,使过去与重要血管神经粘连或包绕而无法切除的肿瘤得到全部或近全切除。残存的肿瘤 可以用CO_2激光予以汽化。我们体会对硬韧的纤维型脑膜瘤无论超吸或激光皆进展缓慢,只在关键部位才应用。对有血寞的肿瘤,二种方法皆不如常规大块切除的方法能减少出血,应慎用。
From March to July 1985, 23 patients with intracranial tumors were excised by Sonotec and CO_2 laser, including 17 meningiomas and 5 gliomas. A case of poorly differentiated carcinoma of anterior cranial fossa.
