目的 观察胃尔宁片对慢性萎缩性胃炎(CAG)脾虚气滞证病变修复及防止恶变的疗效。 方法 采用单盲法设对照组,对照药为维酶素片,疗程8周。两组在服药前后均作内镜及病理活检。动物实验应用Wistar大鼠CAG模型试验及长期毒性试验,后者历时18周。应用狗作胃肠电活动试验。 结果 胃尔宁长期毒性试验表明,高剂量110g/kg)组有轻度兴奋反应,中(5g/kg)、低(1g/kg)剂量无毒副反应。动物CAG模型试验表明,胃尔宁有显著的消炎、强壮、健脾作用,能促进萎缩腺体再生和修复。对狗胃、肠电活动主要起抑制效应,有解痉止痛作用。临床研究435例,其中治疗组323例,对照组112例;治疗组显效以上73.4％,总有效率97.5％,肠上皮化生消失率34.5％,不典型增生消失率30.6％,与对照组差异非常显著(P<0.01)。 结论 胃尔宁片对CAG脾虚气滞证有良好的治疗作用,能使部分肠上皮化生及不典型增生消失或好转,对防止恶变有重要意义。
