目的探讨定量声触诊组织成像(VTIQ)技术的剪切波数值在甲状腺乳头状癌淋巴结转移性癌检出中的效能。方法回顾性分析已明确有甲状腺乳头状癌病史的经淋巴结穿刺活检或手术有病理诊断的105个颈部淋巴结的剪切波速度(SWV)。结果 105个淋巴结有恶性74个和良性31个。良性组SWV最小值、最大值、中位数及平均值与恶性组的差异均有统计学意义(t=6.83、6.97、7.76、7.69,P均<0.001)。依据ROC曲线分析,SWV测值中效能以平均值为最佳,界值是2.695m/s。得出SWV平均值灵敏度、特异度和Youden数值分别是89.20%、77.40%、0.666。结论 VTIQ技术采用SWV平均值于甲状腺乳头状癌淋巴结转移性癌诊断中具有一定的应用价值。
Objective To investigate the efficacy of quantitative shear wave imaging (VTIQ) in the detection of lymph node metastatic thyroid carcinoma. Methods We retrospectively analyzed the shear wave velocity (SWV) of 105 cervical lymph nodes diagnosed by pathological lymph node biopsy or surgery with clear history of thyroid papillary carcinoma. Results 105 lymph nodes were malignant 74 and 31 benign. The difference of minimum, maximum, median and mean of SWV in benign group was statistically significant (t = 6.83,6.97,7.76,7.69, P <0.001). According to the ROC curve analysis, SWV measured value in the best performance with the average, the cut-off value is 2.695m / s. The SWV mean sensitivity, specificity and Youden values were 89.20%, 77.40%, 0.666, respectively. Conclusion VTIQ technique using SWV average in thyroid papillary carcinoma lymph node metastatic cancer diagnosis has a certain value.
