目的 探讨糖尿病患者血细胞体积增大的原因及在微血管病变中的作用。方法 5 2例糖尿病患者部分血细胞参数和红细胞膜 Na+ - K+ - ATPase、Ca2 + - ATPase活性分别采用全自动血细胞分析仪和改良的 Hanahan法、L uthra法测定 ,并设对照组比较。结果 糖尿病患者红细胞膜 Na+ - K+ - ATPase、Ca2 + - ATPase活性降低 ,以合并微血管病变组最为明显 ;微血管病变组平均红细胞体积 (MCV)、平均血小板体积 (MPV)增大 ,平均红细胞血红蛋白浓度 (MCHC)降低 ;而无微血管病变组则正常。 MCV、MCHC和 MPV的变化与红细胞膜两种 ATPase活性降低有关。结论 与 ATPase活性改变有关的血细胞体积增大可能是糖尿病微血管病变发生的重要因素。
Objective To investigate the reasons for the increase of blood cells in patients with diabetes mellitus and its role in microangiopathy. Methods Part of blood cell parameters and erythrocyte membrane Na + - K + - ATPase, Ca2 + - ATPase activities in 2 diabetic patients were measured by automatic hematology analyzer and modified Hanahan method, L uthra method respectively. Results The activities of Na + - K + - ATPase and Ca2 + - ATPase in erythrocyte membrane decreased in patients with diabetes mellitus, which were the most obvious in the group of combined microangiopathy. The mean corpuscular volume (MCV), mean platelet volume (MPV), mean erythrocyte hemoglobin concentration MCHC) decreased; no microvascular lesions were normal. The changes of MCV, MCHC and MPV were related to the decrease of the activity of two ATPases in erythrocyte membrane. Conclusions The increase of blood cell volume related to the change of ATPase activity may be an important factor in the development of diabetic microangiopathy.
